Hey everyone! Im looking for potential treatment for a stretched + ice pick scarring around my mouth area. This is the most bothering scar I have on my face. Please refer to the attached pic.
the status of the scar is that it is caused by a cystic acne a couple of years ago. After it healed it leaves a stretched scarring. Its not a hypertrophic scar per se since its soft not hard. Its more like a stretched skin and is kind of whiteish compared to normal skin. And it feels like that its detached from the underlying skin. Its very obvious under sunlights or any overhead lighting making it look like a bump around my mouth. But its not very noticeable under direct lighting.
I have tried laser and filler which all doesnt seem to work. Im wondering for this case should I go for a excision(I mean to cut the whole stretched skin out along with the icepick scarring around it)? Does being around the mouth area making it harder to heal?

Thanks! The pic may be misleading. The thing that bothers me is not the icepick scars. You can see there is a redundant patch of skin at the corner of my mouth making it look like a bump. So I think TCA Cross cannot help with that.

You have icepicks, some sub dermal tethering in the smile lines(Scarring), and a few rolling superficial scars.
Your case should be relatively easy to treat.
I would do several"cannula" subcisions with perhaps some HA filler to break up the sub dermal fibrosis(smile lines), and work on the rolling scars.
I would also do in the same session tca cross to your icepicks.
In between at home I want you to dermastamp or buy a derminator(google) and needle the scar borders. Also alternate with glycolic peels.
If needed and completely optional lastly do some co2 laser.
Sub and filler will help most + tca cross.
