Cannular subcision can be done with 1-3 holes as they are using that as a fulcrum to treat other areas. IT can be done in 20 minutes. Try Rullan in SD, he only does cannular subcision now. You can do it without filler, even though your were just injected.IT's hard to find a good subcision Dr.

Are you sure about this? According to his website, he still performs NOKOR

"Subcision is performed under local anaesthesia. A 20 gauge needle or a Nokor needle 18 gaugeis inserted adjacent to the scar with the bevel upwards parallel to the skin surface, and moved back and forth in a fan-like motion under the scar to release fibrous bands at dermal or deep dermal subcutaneous plane. A small bruise is allowed to be formed, which supports the released scar, because the blood under the skin helps prevent to fibrous strands from forming again."

It's a dying art, no question about it. But so is open-heart surgery. But doctors haven't abandoned it entirely because people still do need heart transplants, even in this day and age with all the robotic surgeries. In the same vein, Nokor is too important to disregard outright because there's just no comparison between Nokor and Cannula. But what do I know?

@beautifulambitionyou never talked about cannular subcision since I signed up 2 years ago.

I came across an article a while ago probably in late december 2017 that said cannula subcision had less side effects (lumps from hematoma, added scars) than NOKOR. I believe rullan ran into this article and decided not to do nokor anymore. Why does it matter whether its w cannula or nokor? i heard lots of popping with the cannula when rullan did it, and had no lumps to this day, why not use it entirely over nokor? people just assume cuz nook is a mini scalpel its going to do waaayyyy better? idk what do i know I'm not a derma and i don't have thousands of subcisions performed under my belt. if theres an medical article saying 80 something pts prefered cannula over nokor idk why people still talk about nokor being so great. its like the whole tca cross 100% thing there are waaay more negative reviews for it than positive ones probably 5 negative ones for every positive one there is, yeeeettt mods and other people always keep recommending it for ice picks.

@amp2695 TBH I have no experience either way but nokor personally scares me a bit. I'd rather have to go back for more cannula subcisions than risk hitting an artery or something, especially since most of my scarring is on my temples. The fact that nokor destroys everything in its path gives me goosebumps a bit. I'm sure in the right hands it's great but I just feel safer with cannula. Just my two cents.

yea thats pretty much what i said, people still cling onto nokor for some reason idk why. cannula seemed to the same amount of popping tethers.

Regarding Nokor vs Cannula... They are surgical tools. As time progresses so does the usage and evolution of using new and existing tools. The dentist uses different surgical tools, heart surgeons do surgery's different ways.The type of tool is picked by the needs of the patient. It thus makes since that light acne scar patients as I said a few times above would do cannula subcision. Pick the experienceof the Dr and not the tool. Remember Lim still does Nokor for cases, and cannula sub for those with lighter scarring. Nokor can be done very safely if your Dr knows anatomy and has experience.

It's hard enough to find subcision as a dying art many Dr won't do as it's not a big money maker like laser.

I've had 4 subcisions with Nokor needle with Dr Rullan and never had hematomas. I'm interested subcision with cannula needle.

Thinking of getting my 5th one but at this point most of my rolling scars are untethered (disappears when stretching the skin). I dont know if subcision works very well on these. Ive had restylane in the past but was very disappointed because I feel like a touchup is needed afterwards after your swelling goes down.

Seems like it's the same thing as saline subcision, which I've had from Dr Chen (who trained under Dr Rullan) for my first ever subcision. I was very disappointed with the results in comparison to the nokor needle.

I actually had to remind Dr Rullan every time to use the nokor needle because saline subcision is the default. That was 4-7 years ago though.

If it is saline subcision, I do not recommend it. It didnt even work for my shallow scars.

Fillers, TCA cross. I had both done during my first 1-2 subcision treatments but eventually made it clear I only want subcision. He will always recommend more services (more $$) but for me, subcision is the only thing I've had results from.

If I can do fillers now, I can make 75% of my scars disappear.

BA is indeed correct. Several paper showing Cannular subcsion is as effective as NOKOR with less side effects. In a minority of cases, even with intense pressure and technique, using Japanese steel cannulars, scars can not be broken. This is when Nokor comes in handy. A paper will be realised end of this year showing that cannular subcsion has much less haematoma than multi-level Nokor (as a guide, Nokor haematoma rates vary from 4-7% - depending on the extent and level). The trend is most acne scar specialist use Cannular over Nokor- traditional needles due to efficacy, safety and less side effects. More papers to come.

I'll contact Dr Rullan for subcision and TCA cross. I had mechanical dermabrasion in the past with Dr Y in New Orlean (now passed away) and also subcision by Dr Rahimi (not sure I'd recommend)

So is the consensus now that we should stay away from Nokor and only do blunt cannula subcision?

I found a video of Dr. Weiner doing a subcision with a cannula. It looks a lot safer. If I was in Florida I would go see him.

Excactly what is hematoma? Is it just bruising under the skin?

It'sa pooling of blood under the skin.

What color is it? I had subcision three days ago and my skin is turning light greenish yellow now. But I thought that it was expected? And Im told that it will be gone in two weeks

I assume you mean the areas where subcision is done has turned greenish yellow? If so, that's a bruising. That's normal. It will go in a week or two.

Looks like I survived pretty agressive Nokor subcision without getting hematoma then!
