Hi,
I have finally, after so many years, managed to get rid of acne (1 yr of Accutane). Now my face is covered in scars. I am now considering different treatments to try to minimize them as much as possible.
I was offered to have different treatments (Infini RF, Erbium laser, peels, fills, ...) but I would like to hear neutral opinions about my scars. What would you recommend for this type of scars (what type are they?) and what final results can I even expect?
I am aware that I need many treatments to improve the current state, but would appreciate pointing out the 'right direction' so I can avoid trying into treatments that will have no actual impact.
Thanks!
Peter
It looks like you have mostly rolling scars and boxcars. I think subcision for the rolling scars (and filler for any deep enough) would be your best starting point. Most of what you've been offered isn't a bad idea, just depends on the order (and type of peels! Don't settle for less than TCA, which you can do at home). Erbium doesn't sound like it will do anything significant from what I've read.
They all seem quite fresh, how long have you had the scars? vBeam may be an option for the redness.
I have some similar scarring and plan to get subcision (and filler if possible), Infini RF and finally a course of TCA peels following the advice of @BeautifulAmbition who will undoubtedly be along to offer some of his expert advice .
Dont jump straight on the laser bandwagon!
Look at your skin carefully and start getting to know it if your gonna treat it.stretch it with your fingers and look in right light and look at all the dark under the skin scarring, run your finger over it and feel the lumps.
aim is to flatten your skin to its natural state.
you look like you got tethered scarring and ice pick and scarred pores.
subsicion releases tethers, filler raises skin and fills pockets.rf microneedling promotes collagen that should firm up the pockets where filler is as its broken down.
subsice the tethered stuff first,if filler is needed then have filler which i think is the case.get that sorted out first over a space of around 6-9 months, multiple treatments.
rf microneedling will promote collagen production and lift and firm up the indents thats left.space them out every 3 months till your happy and then move onto resurfacing/smoothing your skin with tca peeling.
gonna take at least couple years and it all costs as its pro work.

Thank you for your comment. They are not that fresh The biggest ones are there at least for 4-5 years (period in which I had the nastiest and deep cystic acne).

I don't mind time (couple years) and money. I am aware that I have severe scars and that currently it is impossible to remove them completely. That is not my goal anyway. I am just trying to see what is the best path to take in order to avoid unnecessary time and money loss by committing to treatments that will have no impact on my scars.
Even if someone says - your scars are too severe - there is no help now (wait for SkinTE or something like that), that is ok. I can live with that. Just trying to avoid useless treatments, I guess... (Based on my experience if you go to clinic A that has laser/device/treatment X - then they will recommend X as a silver bullet for your scars, this is a reason why I want neutral opinion)
My plan was to take Infini RF (at least based on my latest research I think it can provide better results for my skin and scar type than lasers would?) + subincision few times and see results and how to proceed.
Every working method for scar revision works,of course as people learn and develop and test new devices/proceedures we are going to get new more efficient methods.
my point is,you got treatable scars,if you are prepared to start the journey then start the journey as theres no time like the present.
If you want to treat with Skin TE (which doesn't really exist yet for "acne" scars) then that would be a discussion in scar less healing, ... we help people who want to treat their scars now.
Your scars are more textural: You have all scar types
Your biggest concern is the redness and this is treated with vascular laser, this would help blend the scars with the surrounding skin.
You will need subcision, with filler if they are a pencil eraser or pencil rubber (tip), if not you can do saline injections and Chinese cupping (amazon).
RF Microneedling, several treatments .... a few brands are fine as long as they are insulated.
TCA Cross for your icepicks (several of these).
TCA peels for your surface texture, many of these, or a deep sedated peel with a Dr. Many Dr's do laser instead these days, but there are more side effects and cost. You can save laser for last if needed, peels work, they just take time.
See the FAQ - top of the acne scar solutions sub - first post.

Hi, sorry - did not mean to sound disrespectful. I know that SkinTE does not exist and I appreciate your advice on this subforum.
Regarding the treatment options that you named - is there any specific order that you would recommend to gain more end benefit or it should be the same to start with any of them?
Oh I know man, no worries ... read my recent post on page 2 of Just Some More Hope post if you wish. I embrace new treatments but I don't think it's healthy to rely on Skin TE as a solution at this time. Think of it this way. If it works in 2+++++ years then great you can only get further improvement right.
Order. Start with subcision and filler, then do Intracel, then Another Subcision if you need a top up on the filler, finish your other intracels, do more subcision when the filler runs out.Treat For TCA Cross when you have time (you will need several). Do Several TCA peels until you get to where you want to be, some do this yearly as upkeep for aging. You can also get a Dr sedated peel to go deep under the texture.
Vascular laser at the end part somewhere for the reds.
BA 😉
