I have brown skin which is highly prone to pigmentation. I am currently getting PRP+microneedling for my scars. I used adapalene gel for a few months before my first session, so I didn't get any extra PIH whatsoever, but after that the doc told me not to use adapalene as there wont be much gap between treatments, so after my second session about 10 days back, I got unusual bruising, the skin turned purple-black and I was given a cortisteroid for that, the bruising has subsided but I am still left with quite a lot of pigmentation which wasn't there earlier. My skin feels very nice but looks awful because of PIH. How should I manage this in between treatments?
Are you sure it is actual pigmentation and not bruising under the skin that is causing a discoloration of the skin?
You could try hydroquinone (a skin bleaching cream) but I am not positive of its safety on open wounds- I would say ~5 days after microneedling it would be fine to use on pigmented spots.
Off-topic: I know it is too soon to tell, but how is microneedling going for your scars?
I don't understand why people worry so much about pigmentation it will fade in a few months. If you want to get rid of it faster ask your dermatologist for different lasers, for example ipl. You can always cover discoloration with some makeup (won't do much for tethered scars), even if you are a guy, because who the heck cares? and nobody has to know.

Idk really... The over the skin bruising has definitely gone away but under the skin?..
I think hydroquinone would be too strong right now.
well, its been more than a month since I had my first session, but right now I cant really say anything because there is so much pigmentation, I rarely look in the mirror. but after the first session the results were amazing, I could clearly see difference in my forehead texture(which was quite bad) and those crater like scars, they responded the most, other than that, not much for the ice picks and the valley shaped scars, sorry I don't know the terms. I think the bruising was due to short time between treatments, I am thinking of spacing them out a bit longer, give more time for my skin to heal.
@Duperele Well for some people it doesn't fade so fast, I have suffered from acne and hyperpigmentation for the past 6 years and these deep scars for the past 2, and when I am actually spending extra money on this stuff and rather than getting any better it gets worse is definitely disappointing.
This is why you do not do treatments so close together. Healing takes time. We have to see how things go.
You were given good advice above, makeup to spot treat, the hydrocortisone which can be mixed with moisturizer if it is too strong and if you had proper healing time in between treatments you could have done hydroquinone before your treatment and retin a (for 3 weeks) to avoid hyperpigmentation and then again after you heal. At this time you must wait till you heal up a bit and start your creams to fade the discoloration.
@eternalrocketYes but if you have acne new pigmentation comes up all the time? doesn't it? I dont belive that hyperpigmentation can last for 6 years. maybe Im wrong.
@Duperele I don't have acne right now thanks to Accutane but yea the PIH has always been there. Its probably been persistent for 6 years because I have noticed that the city I am living in for the past 6 years has always caused me problems, when I go home things start looking better in just a few weeks but when I come back here...same story all over.
@beautifulambition Do you think 1 month gap between two treatments of PRP would be enough? Also, after how many days of my treatment can I start using retin a?
@eternalrocketSure you can "do" one month between treatments. But I don't think this is the best method. Collagen takes 3 months to build / heal. Prp speeds the healing process but the body is still generating collagen months after.
If you wait a bit longer this has benefits of being able to observe how you heal, further collagen building, observing what scars need to be treated. It all depends how your body heals. This is standard microneedling so it can be done once a month but the body also does not heal quickly for some.

@beautifulambition Okay, thank you, I checked some answers on realself as well and they also suggest spacing it 4 weeks apart initially and then later increasing the gap between treatments as maintenance
