Microneedling with PRP made minor improvements, I have LOTS of piting still. Is Bellafill a good treatment for this? Is there much difference? Does it last like its supposed to?

It's hard to advise without pics. "Pitting" is pretty general, and Bellafill only works for rolling type scars (not ice picks). It has it's risks though, because it is a permanent filler despite whatever anyone tells you.

I mostly have boxcar scars but some are rolling and a few ice pick. Here are some pics but not the clearest. What are the biggest risks?

Permanent fillers can cause grandulomas, infection, to remove you must cut and fish them out, they can move as we age (migrate), they are not natural in our body and thus our body goes after them, they are fiberglass balls, your skin will age and sag, but the filler will stay in there somewhere. I always suggest getting HA done to see if you even like filler (it's dissovable and natural), then after a few times you can do bellafill if you so wish.

Permanent fillers have been discussed in detail under the faq top post of the acne scar sub, goto fillers and there is a link. With NOKOR (type of needle must be this type ask specifically) subcision and a HA filler you will get improvement without a permanent filler, from the bruising and tissues that build up, it will take multiple procedures, do a few subcisions then fill,... you can do them once a year for a while then after.

Thank you so much! I read the FAQ not sure why my doc is recommending the bellafill as a first line, apparently they treat lots of acne scarring here and she was recommended to me by other derms in the area. I read the FAQs but am still a bit confused as to whether to keep up microneedling at all, I know you said its more maintenance and it sounds like CO2 laser is typically just the last thing to do just to even things out correct? Is microneedling generally better than than and I know you said years, she recommends sessions of 3 at a time so I'm not sure how often to even go back for that but clearly it sounds like I need to do the subscision and filler first correct? Sorry I'm so confused, very much a newbie and my scars are very difficult for me to see daily and deal with so I'd like to hold out more hope that in years they could be corrected somewhat. I've had them for 5 years now.

Don't worry about microneedling or bellafill. Get subcision done and try the ha filler, or wait till you do a few and then do HA filler.

Microneedling is great after you baseline with subcision, as upkeep, not as the main course.

Subcision will help you (several), filler will lift the spots, and tca peels will help with texture issues.

That is your plan 😉

If it was my face (we only get one) I would not do Bellafill. Dr's love to push this, it's like those tattoo eyebrows, ... they can look so ugly on some people, why would you get that if you can do something more temporary to try it out, see if it's something you like, if the feel and look are ok, especially with the side effects. If you like HA (remember you can inject once or twice a year (depends how long it lasts in you) then consider bellafill if that's your decision. Why go permanent first. Tell them no thank you I would like HA filler only. you can do subcisionand no filler to start and then fill when your ready, this avoids swelling and gives a better look if the body is not puffy from subcision. I personally don't like when Drs do everything at once in a procedure.

I would never do filler and subcision the same day (swelling / puffy) unless your put under for some operation.

It's all on you to choose what you will and will not do. Of course puffy makes things look better. If he injects while puffy you could find Easter Eggs (sarcasm) appear all over your face. You do not want to inject while puffy. Wait till at least some of the swelling goes down, ice it, all that. Come back in a week. It will not be tethered and if it is guess what your doing 4 >Cannula subcisions. Some wait for their second subcision to use filler after. Nothing is going to attach in 7 days, and if your body does do this (I highly doubt with Cannula subcision - bruising), then next time get the filler done sooner after your subcision, just not the same day. They will have no idea where to place the filler or how much, I have had people had to get the HA dissolve because they flew in, did everything same day, and got bumps fyi. The doctor can give you advice / meds to make the swelling go down quicker as well. If you want to do it the same day that's on you, ... please don't post and tell me you got bumps and had to have it dissolved as you have been warned. If you don't swell then they did not do a good job subcising.
