Hi, I just got off 10 months of accutane and my skin has improved dramatically. But like a lot of people on here, I was left with pretty bad scaring. I plan on getting fraxel next summer with a reputable doctor and I have a few questions about this.
1) What type of scars are these (box, rolling, etc)?
2) How would you classify them in terms of severity?
3) About how many fraxel treatments would it take a good doctor to get my skin to an acceptable point? (I know "acceptable point" is a bit subjective, but in your opinion).
Thanks for the help.
What kind of fraxel are you looking at? Unless its fully ablative, don't bother.
Lasers get alot of hate on this forum due to their price and peoples over expectations. Fraxel is not amazing for deep pits, however there is nothing better than lasers for rolling scars in my opinion.
To get the best results you want a combination of therapy's to treat different types of scars. To give you an idea I'm having ipl, then an erbium laser and fractional co2 laser all in one treatment to deals with my texture and colour. After that I'm going to have subcision and nd:yag/ipl for the redness in a few months. so as you can see it takes a fair few treatments and a fair few sessions to get desired results and most of all, time. That being said, c02 laser is expensive and you may want to try other avenues first.
I'm not sure why you would wait till summer to have laser/any scar treatment done. Are you at college/university is that the issue? Otherwise id get it done withinthe next 2 months.
You got more ice picks and boxcar and they require tca cross and tca peels.subscion,i dont know but laser at this stage of your scarring is more likely not recommended.
you need to wait 6 months for your skin to be normal now.also you got red skin or is that the light? Suggests your skin might still be healing so you might get some more improvement without any treatments.
but tca cross is gonna work wonders and tca peel will help with more shallow stuff.

Wow, I definitely need to do some more research on treatment options, haha. I've been looking into fraxal, chemical peels and vitamin C supplements. Doc wants to wait a year before starting any scar removal treatment options in order to make sure the acne doesn't come back and redness goes away to some extent. That said, I think I'm going to switch doctors to someone who specializes in scar removal and see what they think. Hopefully I'll be able to get a treatment done this winter after the semester ends.

Yeah, I still got some redness. I got off accutane in July, so I have some time to heal still. I'm more concerned with the boxcar scars than the icepicks. But yeah, I think I'm going to try chemical peels and vitamin C supplements before looking into laser options.
Dont use topicals with vitamin c while treating your skin,i think.they can be a little aggravating and on skin that is healing its not good as it irritates your skin which is trting to grow and mire likely thin, especialy when its tca peel.
i wouldnt say no,dont take oral suppliments but just a realy good basic acne friendly diet that works for you.
give your skin a but more time to heal, just use a nice moisturiser that works fur your skin andthat will aid its growth and healing.get lits of sleep and stay out the sun,get your hood up and cut down the light you let in your room.stay stress free!
They gave you good info above. I just want to give you some resources. There is a faq top of the acne scar sub - first post. Read about subcision, and tca peels, click the all things peels link for how to do it. I see boxcars, and icepicks as quad said. Your boxcars could use some subcision and spot peeling (TCA) on the cheeks. It will take several of them. Get the subcision done locally to you by a derm. I would not do Fraxel, ... it's overpriced and does not work well even the ablative co2 version Re:pair. Try these treatments during your school year (many treat for a few years when they have down time for the peels, etc). Acne scars are not one and done. You can always do your laser after you try everything else. The things I offered do not have bad side effects like laser and do not cost $$$$ giving Drs bank. Your scars are not severe at all and will improve with the above treatments over time, alternate them by a month apart. They can be done why you are at school again or on break.
Right side is mild, left side I'd say moderate. I'd say it's always smart to work up in terms of aggressiveness. Try TCA Cross for ice picks, subcision for the deeper rolling scars. If you do lasers at all, forget Fraxel. When people refer to "Fraxel" they almost always mean the non-ablative kind which is obsolete and does nothing for acne scars. Fraxel Repair is the co2 version which is ablative (removes the top layer of skin) but even this particular brand is pretty outdated. If you must do co2 lasers, I think Lumenis Total FX / Deep Fx is the most advanced co2 laser on the market. But instead of diving head first into the most expensive laser treatments, you may find good results with subcision and TCA cross.
My personal favorite is fillers, because you see it immediately. There is no waiting around for so-called "collagen production" with lasers that may never actually happen. Think about it. The fastest way to lift up an indentation is fill it, not to sand down the good skin with the bad skin. Why not treat just the scars focally instead of a scorched earth technique like lasers? Lasers really should be left to the end as a final polish to blend things.
