Hi! I have this deep boxscar which is about 0.7cm long and 0.5cm wide, it's located on my left cheek and was the result of some very bad cystic acne. Anyway I read about tethered scar and I'm a bit confused. Like when you pull the skin in one direction, if the scar doesn't even out it is tethered if it does even out it's not tethered. Is this correct? My scar evens out when I pull the skin but you can still see a little little bit that there is a scar there if you look closely. So I'm not sure what to call this scar.
I'm planning on doing TCA cross on it and if that doesn't improve and raises the scar I will probably have a subcision performed.
Cheers!
Pulling in one direction does nothing 😉 Tethering has nothing to do with pulling. Tethering means there are fibrous bands pulling the skin to muscle or fat. The pulling is to see if you can have subcision, if you pull with 2 fingers outward and it disappears your a good candidate. Otherwise its fat deficient and you need more than just subcision. Potentially filler and resurfacing. For instance a small ice pick is a tethered scar that goes very deep (sometimes to the bone), pulling nor subcision does anything. TCA cross is used to make new scar tissue that has a higher bond with other skin. If it evens out mostly but still is tethered that is normal, it's anchored. Sounds like a plan. TCA takes multiple treatments, over a period of time. I often tell people to alternate dermastamping and tca cross each month for a year once it heals do the next one. Subcision will take 3 treatments.
