Sure there can be some swelling, I believe most results are permanent.
Thats good. I really hope so as I also had a subcision treatment. I'm looking forward to my one month and two month progress pictures. They were taken by the plastic surgeons photographer so they were pretty legit photo's.
In your opinion, how long do you feel one should wait to continue making progress. (I plan on having another subcision done two months post first treatment)
i am not sure. Sometimes i dont get scars or skin. I had three deeper scars and had subscission done with laser. At first the results where mostly from microswelling. Altgough still a lot of improvement. Then about a year later i noticed the scars where a lot less deep. Not sure if dermarollibg did this. How i think about it is that suvscission well preformed cuts the scar tissue between the skin and fat other tissue. The only way for a deep scar to heal is if it is lose so the depression can heal. Deep dermaneedeling might help regaintissue. Or the body just slowly starts to heal from the inside. I think a second subsission only makes sense if the first one has failed or has completly healed. Two months seems slim. Then again i am not an expert. Only thing i noticed with dermarolling is that there is a huge difference between 1,5 mm needles and 2,5. And vaseline essential minerals etc the day after. If i had deep scars i would use dermapen months later as follow up. But that is just guessing i am not the person to ask. I never had a lot of deep scarring. Skin is doing good again
3 week update:
Still extremely happy with the results. Some very minor changes in the last week, difficult for me to even notice.
Next week I will explain the results as I see them and take a bunch of pictures trying to get my scars at the worst angle possible.
I've been suctioning once a day, may continue to do so for a while even though I'm not sure if it is doing much at this point.
4 week update:
The few hard lumps that formed after subcision are all slowly disappearing, only one of them is still visible.Took about 50 pictures today trying to get them to match the before pics as best as possible and show the scars clearly, still not completely happy with all of them. Here is my opinion on how the subcision affected the various scars.
- Left Temple -Slight improvement for most of the scars, although the entire area is still really uneven. Somewhat difficult to see the results in the pictures, but most of the scars here are more shallow. I think one of the scars here was missed.
- Left Cheek -Huge improvement to the worst scars, slight improvement to the less severe scars. Overall extremely happy with the results here. A large area of the cheek is still atrophic, but a lot of the unevenness is improved.
- Right Temple -Most of the scars here were close to grade 1 or 2, but big improvement to most of them. One of the scars didn't improve at all, this is the one I mentioned earlier as being very much a textural problem. Overall very happy with the results here.
- Right Cheek -Decent improvement to here, more so on the area of the upper and mid cheek. Happy with the results.

how are you doing? any updates for us?
Still doing really well, some days I think it looks worse than last week, others I think it looks better. The one lump that has been bothering me is definitely shrinking, it is still slightly visible though. I planned to do another 12.5% tca peel a few days ago, but have a few active spots so I've postponed for now. I'm taking pictures weekly now, maybe Ill post some more at the 2 month mark. I think with 1 or 2 more subcisions and fillers I will have some really excellent results.
How about yourself? The frequency at which doctors do subcision seems to be all over the place, I saw your doctor recommended 6 months between.

I just got back from my subcision. I don't think it matters much how long you wait between treatments so long as you give yourself enough time to heal. So I would wait 10 weeks minimum between treatments. The doctor who did this procedure was one of the most down to earth, intelligent persons than I have met in a while. He was a nice guy to talk to, seemed really knowledgeable and did an awsome job on the subcision (from my own personal judgment). Yeah my head is lumpy, but the scars are smooth and flat. It's strange looking in the mirror to not see the indented scars that have bothered me so much for the last 10 years. As for after care, I'm not making any facial expressions that would move the skin. I don't plan on washing my face for a while, as i really do not want to apply any pressure to the areas that were subcised. I plan to start suctioning on Thursday night, 2&1/2 days after the procedure. I'm super hopeful at this point. Thanks PCT14 for your posts. I had this procedure scheduled for months in advance, but your recent posts have really helped leading up to the actual procedure.

Sounds like you had a very similar experience to mine, both with your scars and with the procedure. Hope everything from this point forward goes well for you.
Glad to help, a lot of people on here who shared their experience with subcision and Dr. Rullan are the reason I finally made the choice to give it a shot. I was planning to have it done months ago and kept procrastinating, difficult to make that commitment after so long.
Looking good!! Right cheek has the most noticeable improvement IMO.
I really, really believe subcision is one of the best treatments for a certain type of scars. Just one won't be enough for many of us, but continued treatments will always take you one step closer.

I just got done suctioning for the first time using the Chinese khangzhu set, with jaboba oil on my face (as Robertitoo recommended) . It seems the suctioning has caused hard lumps to form in the cavities in the dermis where the subcision needle had tore the under layers of the dermis. This is pushing the skin out where the indented scars were. I had the subcision done on smaller individual scars as opposed to large broad areas. There were 12 entry points total. Are these hard lumps dried blood? I know you had mentioned you had lumps in one of your recent posts. I feel like the lumps mainly formed do to the suctioning. Or maybe the lumps were always there and the suctioning helps pronounce (or pull) the lumps out for longer periods of time giving a better end result. Any thoughts on this?

Yeah. They are small football shaped lumps under each subcised area. They only come up when I do the suctioning. I cannot see the lumps, I can only feel them. They disappear again after like 3-4 hours after the suctioning. They won't be needed to be treated with steroids or anything like that. It has been 4&1/2 days since the procedure. Things haven't changed much at all since the procedure. The scars look flat with surrounding skin. The skin is still a bruised purplish color. I'm just hoping the scars can stay as flat as they are now (as much as they can) permanently, and the bruising eventually fades. I Will continue to suction three times daily. I need to go back to work in 11 days, so I am hoping things clear up mostly by then. PCT14, I think you got the best results on the right cheek. There was a large indentation there that seems to have disappeared. You also got improvements in all the other areas. I agree that with a few more treatments, you will see amazing results. Looking forward to any further updates you have. Its strange why there are not more cosmetic dermatologists doing subcision for atrophic scarring. They probably make more money off the lasers. I'm not sure if I experienced any changes after around the 2 month mark, maybe I am just not very perceptive of these things. In a lot of ways I have to look at the pictures I took from before the procedure to remember what my scars were like. Wish I had taken some better pictures, but I only started to get very familiar with the camera after taking pictures of myself daily.
Glad to say I have not experienced any negative side effects. All of the lumps have completely disappeared and I have no marks or redness at any of the needle insertion points.
I dermastamped a few weeks ago and will do a 12.5% TCA peel in a few days. I have my next subcision scheduled in a little less than 1 month. Probably wont update quite as often as the first time, but I will definitely document it here.
Cool! I just got y treatment with Dr Richard Sadove in Gainesville Florida and he does his subcision with auto fat grafting transfer. So far so good. I also got excision though and that is what i am mainly concerned about, are those scars healing but i really trust him. Time will tell. I am at my two week mark. I went in today for a second post op and he had me get silicone scar treatment pads at CVS to place on the excision areas to flatten and reduce redness. I hope your procedure works out with great results!!! It's exciting to look forward from these scars. It is crazy, sometimes i feel like i still have acne and i don't....Strange how it hits you so hard and kind of stays with you even after it leaves. I wish i would have known i was allergic to dairy before all this trouble. Good thing for such advanced procedures now
Ask your Dr about auto fat grafting transfer with subcision...I am curious what they have to say about it. Let me know!

Yeah, the Dr. injects saline after doing the subcision. It's been exactly 2 months since my subcision procedure. I am very pleased with the results. All the scars showed improvement. I would estimate anywhere between 25%-80% improvement on the individual scars. The scars that showed the most improvement were the ones that were more tightly tethered down. People have been discussing on here how it can take a few months for the collagen to rebuild itself, thus delaying the results. I would say that this has happened with me. I have noticed in the last week or 2, that I have seen the scars and skin itself improve in texture and has even smoothed out some more. The redness under the scars where the subcision was done has also mostly faded over the last 2 weeks. I remember the doctor saying that the pools of blood created by the subcision actually eventually turn into collagen itself. As the redness (or dried pools of blood) faded, the scars have improved dramatically over the last week. This is the best my skin has looked in decades. I would advise anyone really struggling with facial atrophic or acne scarring, to look into subcision.

I looked into fat transfer with subcision when I was researching subcision and it seemed like a very interesting treatment, there just was not enough documentation on the results for me to justify the cost difference at the time. I will take another look at it before I set up my third subcision, will also start to take a serious look at fillers as well.
For your two week mark your skin looks greatly improved, I will be following your progress to see how you are doing in the coming weeks/months.
Yes he injected saline in addition to the subcision. I cant say whether this is really much of a benefit or not, but I personally could not think of a reason why it would be a negative. I will take a bunch of pictures a few days before my nexy subcision and create a new album for them.

25% - 80% is incredible. I dont think my improvement is as significant as yours, but I am still happy with the results. Hope my second subcision is equally or more beneficial. Thanks again.
Ill be posting post op pics of my 1 month mark with subcision and auto fat grafting/excision. I am thrilled with the results and the Dr says he doesn't think i will need more. He is certain that wont need more, actually. But he said to see after a few months what i think. ONE TREATMENT! I am so happy to say that it works with auto fat grafting the first time! I don't think i will need it a second time. I can finally stand underneath lights without worrying. I just am thrilled. The first 4 weeks is scary i must say because your skin is doing so much healing and it doesn't look very attractive but i was able to work after one week, just swollen and with silicone adhesives on my excision marks so its a humbling healing process...BUT WORTH IT. Pics in a couple days! I am still healing for the next couple months so the excision marks seem to be taking the longest to fill in and lighten but they are coming along especially since i started wearing silicone scar adhesive pads as Dr Sadove recommended. Good to read that it took a couple months to see even more improvement. I can't wait.
Just got home from my second subcision, process was exactly the same as last time. I updated the 1st post with current info and added a new album. I wont be updating as often or with as many pictures this time unless my recovery differs somehow. I think around the 3 or 4 week mark is when the actual results can start to be seen.
Added some new pictures, today is 4 weeks after the second subcision. Color and angle is not the best comparison to the ones I have in the first post, maybe I'll take some more later or update the original post with some different pictures from my oldalbum. I will also take some pictures with indirect light and add those soon.
As for the subcision and recovery, almost everything was exactly the same. I did not get 3 small lumps like last time, but instead have a single dime size area that is still slightly raised and firmer to touch. It happens to be in the large area of atrophy on my left cheek and only barely visible so it maybe it is actually a good thing if it doesn't shrink over the coming months. I also got a lot more aggressive with suctioning starting around like day 10, no idea whether it had any impact.
Happy with the improvement again, especially in the left cheek. Still have a lot of atrophy but all of the really deep scarring is more or less the same depth as the more mild scars. Will probably wait another 4 weeks and then start some dermastamping and TCA peels.
