Skin Recovering Rapidly! 40Mg Accutane Log

To all the beautiful people here (yes, we're beautiful regardless of acne):

I'm a 26 year old guy who had great skin till 6 months ago. All of a sudden, with no change in diet, I started getting pimples. Not just the tiny ones that disappear but the deep, scarring ones including a cyst (something I never had before). To add to that, some of the breakouts that healed naturally left lots of (15-20) PIH marks. Anyway, after ignoring it for a few months (big mistake), thinking it would just all disappear with a good diet, I bit the bullet and went to a derm. He's one of the most respected and qualified derms in my city and he didn't mince his words. 'You've got a severe case of acne, and the only real solution is Isotretinoin'. The cause for my acne was emotional stress, since I've eaten chocolates and drunk loads of milk in the past with no real breakouts. So there began my isotretinoin journey. My first appointment with my derm was on 2nd December 2013, and here's my story so far.

Month 1: Antibiotics (Doxycycline) for 20days, followed by Minocycline for another 20 days. Then I began 20 days of Accutane and Doxycycline. Controlled acne somewhat but still continued to get pimples. Got 1 cyst which was injected with cortisone. Went down but left a red mark. This was the time I also began Spectra laser treatments for my deep acne marks aka post inflammatory hyperpigmentation. The first laser didn't make much of a difference to my skin at all. But my fingers were crossed!

Month 2: Sotret 10mg. 20 days on it, not much difference. Not even drying of lips. Bad headache, neck and back pain but I think it had nothing to do with accutane since I remember spraining my neck while working out. My derm almost took me off it until I told him about the muscle pull. Then he was convinced it wasn't the Isotretinoin. Thank God, I didn't quit at this stage! Did my second laser treatment. Marks got a tiny bit lighter, but nothing to write home about yet.

Month 3: Sotret 20mg (10mg after lunch, 10 after dinner). Dryness kicked in, with lips being constantly dry. Got 1 cyst, and the derm injected cortisone which flattened it in a few days. The mark still remains, although it is fading well. Blackheads and whiteheads from my nose disappeared completely. Lips began to normalise after 10-15 days. Got my third spectra laser treatment. Marks DEFINITELY did lighten according to most people who saw me, including the derm but when you're monitoring these marks daily you tend to not notice much of a difference. I continued to get a couple of pimples here and there but they were not the kind I used to get earlier. Smaller and would dry up fast. Did get one bump next to one of the hyperpigmentation marks but the surrounding skin is white which makes me think it's collagen growth and my skin repairing itself, not a pimple. Anyway, the bump is almost gone.

Month 4 (current) month: Sotret 30mg (10mg in the day, 20 at night): Been only 14 days on this dose, so not much change yet. Dryness seems normalised, and there's no oil on my face. One pimple on each cheek but they dried up in a few days.

So not counting the antibiotics month, I've been on Sotret for around 56 days. Apart from the back pain (which was really painful but quickly disappeared with painkillers), I can safely say that there have been no side effects. While I don't mean to discount the experiences of those who did get a lot of side effects, I believe that many of these are hype or your mind playing up. I have not even suffered a nosebleed. However, I must add that I am an extremely positive and grateful person, and simply believe that this medication is safe and effective for me. Trust me, being happy goes a long way in curing your skin problems. Also, there's a lot of talk about not doing laser while on accutane but there's NO research that supports this theory. Anyway, my derm believes it's safe to do the specific laser I'm doing (Spectra) even when on Isotretinoin. Also, if anyone is undergoing laser and feels their marks get worse and much darker after treatment, don't freak out! This is completely normal since the pigment comes to the surface. The marks will look lighter only 1-2 weeks after treatment.

I will continue to update you all every few days so that more people on Isotretinoin can benefit from reading this. I believe Sotret is no longer available in the US ( I live in India) due to some manufacturing issues, but my derm believes it's safe and effective. He did give me the option of switching to another brand, but since I haven't experienced any major side effects from Sotret, I'm happy to continue on it. Although I'm sure that regardless of brand it's all the same thing. Moreover, my derm himself said that he suffered from horrible cystic acne and he was on a super high dose of Sotret. Nothing seems to have happened to him, and he's got a thick head of hair! And his skin is flawless, which makes me super hopeful that I will get there soon too. Post accutane, my derm will be doing Fraxel for the really stubborn marks, but there's still time for that.

While I'm sad that my skin went from great to this in just a few months, I know many people here suffer from greater acne issues. God bless us all, and I truly choose that no one suffers from acne EVER. I will write soon, and meanwhile, please feel free to ask me as many questions as y'all want on this thread. I'll be happy to reply.

Okay. So it's Day 58. My marks seem to have lightened up dramatically over the last 2 days. Isotretinoin has kicked in totally now. Dryness everywhere. Feeling a bump under the skin on my right cheek. Pretty big. I'll be showing it to the derm tomorrow (12 hours from now). Hopefully it's just a normal pimple, not a cyst. Will update.

Its crazy how a perfect diet and following every single anti acne regime didn't help at all. Thank goodness something like Accutane exists. I sort of got lazy with my blog.. With no real replies from the community here. I will try and put bi-monthly updates up though.

Im on day 30 now.. My lips are always dry now, so I am using blistex medic on it quite frequently - Which has been a life saver. I currently have no active cysts on my chin (touch wood), which to me is just amazing! but I have one monster on my upper lip, thankfully for us guys, we have facial hair to cover it a little.

Thanks Ben. I totally agree with you. Accutane is truly a godsend. Don't worry too much about the bumps on the cheeks. I've got those too but they quickly disappear as accutane begins to dry you up completely. Also, I recommend shaving daily on Accutane since the constant exfoliation certainly helps. That's what my derm said.

 

 

Update: 9 weeks Isotretinoin.

Hi all

I had an appointment with my derm today. He was really happy with my progress. The bump on my right cheek that I feared was a cyst turned out to be just a regular pimple, which had already dried up and become tiny when I woke up. I can barely feel it now.

My PIH has faded to the point that my derm didn't feel the need to do laser today and said that he might do a final one after 3 weeks IF the marks don't lighten naturally by then. Also, he told me to continue with 30mg for the next 3 weeks, after which he will increase it to 40mg, which will be my final dose. So I have only about 2 months of Isotretinoin left

In my prescription, he added a multivitamin capsule, Vitamin D Sachet, Vitamin E 600iu, and a Vitamin E topical ointment (in capsule form) that needs to be mixed with my moisturiser at bedtime. That's what I love about my derm. Along with Isotretinoin, he also suggests other things that help speed up the healing process

My skincare routine at the moment is perfect. Let me know if y'all have questions

Okay so I had a bit of a scare today morning. On one of my marks, there was a raised bump. This freaked me out since the last thing I need is pimples on my marks, especially when they seem to be finally healing well. It looked quite big but then suddenly seemed to calm down over the course of the day. Thought it was a pimple or cyst but turned out to be just a random bump. Called up my derm who said it's either a tiny pimple or just some inflammation from skin rebuilding itself and I should let him know if it doesn't subside. I'm assuming it's the latter since it doesn't hurt at all. Just feels like a little bump now. Thankfully it's gone down a lot. Hopefully it'll say goodbye tomorrow.

Phew! it's funny how every little skin change gets us paranoid.

Okay I've had another small (really tiny) bump on one of the existing acne marks. I'm not sure what's going on but I only hope it's temporary. Derm still says nothing to worry. The marks however are continuing to lighten :)

Okay so I spoke to my derm and he said that I need to stop the Vitamin E cream since that was the culprit. I did get a couple of more pimples and a biggish under the skin one on my right cheek. Right next to that is another much smaller one. Sigh. Just when I was making some real progress! Anyway I am continuing to be positive and just think that all the acne is finally saying goodbye for ever.

I'm in same boat.. Skin took a little turn for the worse over last couple days.. Trying to be positive too. Also the sun is really taking its toll on my fair skin too! Sunscreen to me right now is vital!

I had a mini breakout over chin.. maybe 6 little whiteheads and 2 inflammed pimples. I am blaming fish oil tablets for it at the moment.. hahahaa its the only new thing i have added and skin went from being clear of active acne to this.. so its all a bit of trail and error.

Im a bit lost at the moment with my skin routine.. to may options and things I do.. and Im not 100% sure which things are working and which ones arent..

Stop taking the fish oil immediately. My derm said that it can cause breakouts, especially if your skin is sensitive. I agree that everything is trial and error. The vitamin E cream that my derm gave me worked on all his patients except me, so I guess we're all very different when it comes to skin.

I had a nasty food infection a few days ago and had to stop Isotroin for a few days. However, My skin has been looking quite good since last week's Vit E breakout and right now i can say that there are no ACTIVE pimples on my face since a week. Fingers crossed. I got bumped up to 40mg last week so maybe that's why. I also switched Isotretinoin brands, but i doubt that makes any difference. No side effects at all thankfully. I have an appointment with my derm on Saturday for my 4th round of Spectra laser, which treats post-inflammatory hyperpigmentation.

I hope we get acne-free super soon.

Yeah I took your advice, but maybe a little late.. I went from no cysts to having 2 on my chin in 4 days of taking the fish oil supplements. Now I have 400 Capsules that I will donate to my Brother hahaha.

Hows your accutane journey coming along?

I spoke to my dermo over phone yesterday (to review my latest blood work). Last time my liver enzymes were borderline, but this time he said all of my tests were absolutely bang on perfection, so this has brightened my weekend. I asked him also about my treatment plan if I would step up from 20mg a day through the course. He said at this stage no, but we can speak about it at our next appointment, which is like in mid may. I have been on 20mg now for 2 months exactly as of today. Whats your thoughts on stepping up with the daily dosage?

Hi Ben!

I'm sure your cysts will clear up really soon. Did you speak to your derm about getting them injected with cortisone? That's what I did and they always went away the next day. Of course, they always leave an annoying red mark that takes a few months to disappear.

The journey is going really well so far. Had an appointment with my derm today and he was really pleased with the progress. He is really caring but barely ever smiles and he almost pulled off a half-smile in happiness. So now I know I'm definitely on track. Haha.

I personally think that waiting till May to increase your dosage would be too long. I've seen a major improvement in my skin this week, and I believe that's only because of doubling my dosage. Talk to your derm in a couple of weeks (or before if you're getting your cysts injected) and see if he agrees to bump it up a bit. Maybe 30mg for a while and then finally 40mg. Great to know that your bloodwork is perfect :)

My Accutane course will be over by end May, until which I'll be on 40mg daily. As for the post-inflammatory hyperpigmentation, I had my 4th and last round of spectra laser toning today. My doctor expects the marks to lighten tremendously by the end of this month. If not, I might need 2-3 Fraxel treatments, each spaced 21/2 months apart, 1 month AFTER my accutane course. But I'm positive that I won't need it.

As for writing on my log, I'm super happy and thrilled that you're doing so. Certainly continue. It's good to have someone on the exact same journey!

Thanks so much for the advice! Unfortunately I dont have the option of cortisone injections at my clinic - I could source it elsewhere, but for now if they do pop up I just try and be as careful as possible.

In the last week my beautiful mother got remarried and a few other highly stressful events - so Im linking that to why my skin is paying for it this weekend. No BIG cysts, but alot of small blind pimples that as you know take a week to finally surfice and then are prone to red marks and such for a month hahaha. Trying to stay positive.

As I was saying - I called my derm (who is more business like, than supportive) - and he said we will discuss in May. So in the meantime ive taken.. well not really a gamble, and I know I most would think its stupid, even myself - but im taking 40mg, then 20mg, then 40mg, then 20mg - as to hope it rounds out to about 30mg a day. Only took my first double dose this morning, but the fact that im 2 months in and still problems really havent significantly change, has led me to think this is a wise idea. Please let me know your thoughts. Obviously come my derm appointment in mid may, Ill sit down and take his advice on what dosage, but from what I can tell with accutane 'if you are handling the side effects well, and you havent yet seen significant results after a couple months - most doctors will up it a little as trial for a month or so'

Hi Ben!

My course is going great so far. Lots of dryness of course, but that's to be expected. No new pimples since 2 weeks now so I'm pretty kicked. Except for 2-3 tiny forehead ones that resolve in a day. The red marks are still around, but are definitely fading. The marks are the most annoying since they take the longest to heal, and even though you're technically 'clear', you certainly don't feel it with these around. I've added pycnogenol supplements to my routine since there's research suggesting that it can help to lighten post-inflammatory hyperpigmentation.

Congratulations on your mother's wedding! I hope you had a good time. Sorry to hear about those cysts. Whenever I got one, I just rushed to my derm. The city I live in is really convenient when it comes to medical care. It's never hard to get a derm appointment. And there's never a delay. Just need to call a day in advance. I don't think there's anything wrong in upping your dosage, since I too had to coerce my derm into increasing mine. He wanted me to stay at 30mg for another 3 weeks, but I insisted that I want to be done with my course soon. Higher dosages equals rapid clearance. Although I would still suggest break up your doses - like 20mg in the morning and 10mg in the night instead of alternating between a once-daily higher and lower dose. I'm not sure whether it matters but I'm just saying what worked for me.

Whatever you do, just don't stress. I know this is the hardest part, but stress does make things worse. Find something you love doing, and try keeping yourself a little distracted. Remember that Accutane takes a while to show results. I am seeing somewhat good results only now (And I've been into my course for over three months). Some people clear only at the end of their course. And some after. So it's really different for all, but rest assured, it will work.

No, I don't. I'm too conscious of it at the moment anyway to share on a public forum. I even avoid looking in certain mirrors! Imagine posting a picture I have one when my acne was at its most severe. I'll put both up when my skin is near-flawless.

Okay so I broke out a bit on my forehead and I'm convinced it's due to the Pyncogenol supplements since that's the only new thing I added to my routine. Also, I never breakout on my forehead so it has to be the culprit. The same thing happened with topical Vit.E too so I'm not making a mental vow to not add anything new or fancy to my routine. My basic regimen works well. I'm also stopping all supplements. I think for us acne-prone folk, a lot of stuff is eliminated through skin and we should be as simple as possible with our routines.

Minor breakout aside, my skin has been looking better but obviously nowhere near where I want it to be. I've noticed that my skin usually looks very good in the morning when I've heavily moisturised the night before, so I will be doing that more often.

Let me know if anyone has any questions. Also, I'm so glad I went on Accutane. God knows how much worse my skin would have been by now if I hadn't. About 2 months left for my course.

Well so I have some great news and some not so good news. My derm today said that I no longer have PIH. Yay! Basically the spectra laser got rid of all the dark pigment and now what's left are just faint, pinkish marks. However, the bad news is that one of the marks has given way to a slightly indented one, which was earlier hidden due to the discolouration. I'm pretty freaked out that the other marks might have indents below them too, but I'm just staying positive in the hope that it's just one. My derm is confident that Fraxel will eliminate even these scars (2-3 sessions) but I believe that I might not need it. I want to give my body a real chance of healing itself before I subject it to a somewhat risky procedure.

Break-out wise, I got two pimples between my eyebrows but they have subsided. Nothing major. Apparently these can be caused by wearing spectacles! Which I've recently started doing so who knows. Overall, my skin has been looking quite good of late, and if it continues to improve like that, I might just let things be rather than doing something for the sake of vanity. I have had a pitted scar fill in once and I believe it can happen again. Anyway, Fraxel is at least 4 months away should i choose to do it, so no point thinking of it. God might bestow a miracle and I might not need to do it at all!

Hey Rohan, I was just wondering how your skin is doing now. Did you ever have any problems with the laser treatment while you were on accutane because I know in the west they advise you not to have any type of laser treatment.

Hey guys. Really long delay in replying since I was travelling with no net connection. I recently completed my third fractional laser treatment and results have been great so far. I have a few minor indents which seem to be filling up quite nicely. I started the fractional laser one month post accutane and there have been no problems. I think the negative hype against lasers is due to people getting it at spas etc instead of a proper dermatologist's office.

I still get breakouts but they are far more manageable. Let me know if ya'll have any queries. Will be able to reply more frequently now.

What kind of fractional laser did you have? I also keep hearing wait 6 months but I really don't want to. There seems to be no proof as to why. I'm only 2 weeks post accutane
