I'm baaaack (with scars)

Hi everyone,

Some of you have been following my journey with acne in the past. I haven't posted in a while, but if you are interested, I can update my Log (linked on my profile) with how I got my acne under control.

I'm here because I don't have much knowledge on scars, and I am hoping for some recommendations. I took some photos of my skin in the WORST lighting possible - I know in other lighting the scars are not as noticeable, especially with makeup. However, I am also experiencing loss of collagen due to getting older, and cannot use retinol or anything harsh because my skin is too sensitive. I can't even take collagen supplements because they break me out due to histamine intolerance. So, my options are limited.

My facialist recommended microneedling. My skin does not heal properly (e.g. oftentimes injury scars don't fully heal or leave keloids) so I am worried about doing anything too intense.

Suggestions?

I also wanted to point out some other issues, circled:

- The 2 larger circles are where my skin has large indentations - it's as if my skin has sunk in and looks atrophic.

- The smaller circle is just an example of little spontaneous scars I have been getting with no apparent reason.

- Large pores (cannot use salicylic or other acids besides lactic)

First off you're scarring is pretty minor imo so thats good. I'd say your scarring is mostly textural on the cheeks (very shallow/minor)with some atrophy on the chin. For the cheeks Id say resurfacing techniques would be best but as you said you're skin is sensitive so that may be an issue. Lasers maybe an option but I'd be very careful if your skin is sensitive. Another thought is that texture can be heavily influenced by your skin barrier strength. Since your skin is sensitive and can't tolerate many topicals I'd say your skin barrier may be slightly impaired and you could see some textural benefits by strengthening it.

 

As for the atrophy on the chin I'd say subcision/filler would be the go. Its pretty well tolerated even if your skin is sensitive, but there are some risks with keloid scarring I believe. I have had 4 subcisions withvery sensitive skin and my skin improved a fair bit.Don't take my word for it though, I'd recommend looking into it and consulting a professional of course.

Microneedling is a poor recommendation, especially if your skin isn't great at healing and is sensitive imo. Even if your skin was great at healing, I have yet to see many micro needling that helped much. If you mean the indentations only then what would micro needling do?

The cheeks are a tough call. You have some textural stuff, but everything is level generally. Your options are laser, peels maybe (less effective) or some other stuff I guess, but you risk turning a small problem into a large one. I welcome another person's suggestion though on that one.

As for what I'd suggest for the depressions, the doctor I went to a consult for 6 months ago swore he's done hundreds if not a thousandof fat filler transfer that he's not seen any adverse affects. I tend to think the procedure itself is more for atrophy or aging than it is for acne scars maybe, since acne scars have broken layers that might be unpredictable with clumping. But if your concern is the two indentations near your chin then it might be something to research. The average cost is around $2750 and they just take fat out of your tummy/butt and spin it, and inject it in depressed areas.

The other things are more temporary fillers.

And bad lighting or not I think you are pretty, you're braver than me I get ptsd if I get near something that takes my photo.

thanks for the feedback. That's exactly my worry: turning a smaller problem into a larger one. I don't know much about fillers, but I will look into this. They are very expensive though. How long do they last?

Good suggestions. Yes, texture is definitely an issue. The only peels I can tolerate are lactic ones, and my skin looks better for a few days after, but then I return to clogged pores and uneven texture. I rarely wear makeup and clean my face regularly, so I don't think it's an external issue. Any tips on how to strengthen my skin barrier?

Have you posted any photos of your progress with subcisions? How was the healing process?

Have you posted any photos of your progress with subcisions? How was the healing process?

One thing that might seem counterintuitive is to give your skin a rest from harsh topicals (even the lactic acid). I'm still dealing with texture issues from harsh retinoids and its improved significantly by forgoing all topicals and eating/sleeping well. It could be the case that once your skin barrier improves the added hydration will reduce the amount of oil produced and plump up the skin more.

 

I'd be happy to dm some picture of my subcision progress. I generally took 4 days to heal from the swelling and about 11 days for the bruising to go away.

I'd be happy to dm some picture of my subcision progress. I generally took 4 days to heal from the swelling and about 11 days for the bruising to go away.

I have done a lactic peel last week, after 3 months of not doing anything besides moisturize and home-made face masks. My skin is very dry. I generally have a very minimalist approach to skincare by default, but yeah, I will try to just let it do its thing.

Fat filler transfer purportedly lasts 5-10 years but could be subject to "weight loss" so it's suggested your weight not fluctuate much they say. The other fillers that are safer like juvederm most say lasts 1-6 months (pathetic) and the more permanent fillers apparently have more risks and may not be applicable either way. I mean one filler has small metal things in it that don't dissolve so I just never liked the thought of that.

My scars get more noticeable with weight loss/ collagen loss and age. Yes, I am very cautious about fillers...trying to see if I can improve the situation gently, but it seems that more intense treatments might be needed.

My scarring is very similar to yours. I am going down the laser road at some point. Going to do the Total FX laser and fingers crossed I might get some improvement with that.

As far as your chin scars I have to wonder if that is actual scarring or not. May be filler might benefit that area but too me it doesnt seem like actual scars. I guess like dimples but on your chin.....if I making any sense....

Hello,

I still take DIM but now I don't need it as much, so I only take it when I'm PMSing, right before my period until I start spotting. Initially, I took about half a capsule (from the regular 100mg) every day until my skin cleared. This lasted for about 6 months, but I saw improvement almost right away.
