Would subcision + filler treatment suitable to my acne scar

Hi guys,

 

Would like to know whether subcision + filler treatment help with my acne scars revision? I done few times of CO2 laser many years ago. However, I didn't notice any improvement. And recently I've been reading good reviews about subcision + filler. Would like to know whether does it help? My scars are mostly concentrated at cheek + temple area.

 I can't really make out the depth of your scars. Can you please make another snap using the suggestion as follows?

Consider taking a picture using the angle-lighting approach like the one shown in the picture below. This will create a heavy shadow that will accentuate the scar for better evaluation. This can be done by pointing a bright light perpendicular to the angle of the lens. So if you're shooting the side of the face, you would point the light from either the front or the back of the face.

It appears that you have a combination of boxcar and rolling scars. They aren't severe, but they're widespread. I don't think it would a bad idea to start with subcision. After 3 months, I would proceed with TCA to iron out the hard edges.

If you plan on getting filler, it should hyaluronicfiller like Juvederm. Also I highly recommend youwait at least 2 weeks for the swelling to subside completely before injecting. Make sure to raise the pits as much as possible.

But 2 weeks after subcision without filler , Dont scars retether ?

No. Scarring is a slow process that can take up to 4 weeks. It doesn't happen overnight.

I understand it doesnt happen overnight we are here talking About 2 weeks, but people start this suction technique when dr doesnt use filler on 3rd day after subcision to prevent retethering

In that case, what does "retethering" mean toyou? When the scar fiber is surgically severed, you intentionally createa wound. Like any wound onour body, that wound needs to be healed as well. The healing process is very gradual and can take up to 4 weeks.

Why you should wait at least 2 weeks before injecting filler is due to swelling. But can the scar re-tether in that time? Likely not. Also when the filler is injected, they usually do a light subcision (eg. lancing movement) anyway. That will break up any possible retethering, so there shouldn't be any concern.

You highly recommend injecting the filler at least 2 weeks after subcision based on what exactly? Is there scientific evidence (papers) to show it gives better results than subcision and filler in one go? Which scar specialists do it the way you recommend? Thank you.

It's more common sense than "scientific evidence". How could you realistically and precisely inject fillers when the face is so swollen, which is always the case after subcision? Do you really need to a write a paper on this for people to understand?

So youbase your recommendation solelyon your subjective opinion, because it makes moresense to you?No medical papers, no proven to be effectiveways of doing it from the top acne scar specialists??

Of courseyouneed papers that prove something is working. It's how clinics and doctors choose and applythe best possible therapies, based on scientific evidence. Otherwise every qualified or not so qualified person would do what feels like "more common sense than scientific evidence" to them.

It's really too bad you are not a doctor and don't have a dermatology clinic( I'm guessing since you're posting here and usually doctors aren't allowed to post). You and your common sense would have cured all our acne scars by now. Why do to the top acne scar specialiats anyway? They've been doing it all wrong relying on evidence and experience ahaha.

Scarring starts to retether again on the 3rd day.

the problem is that your face will swell after a subcision, so depending on the doctors skill, they may mess up. I would get them both at the same time personally.
