What can I do more to improve my acne scars

@HuskyRayon Can I ask that you be patient with me. Just one of me, I am volunteering here to help you and as such am not paid like the ask a question tele-derm services. Both of your questions are the same and have been combined. It will take me 24 hrs to respond to you, I help well over 100+ people a day, many private messages, etc. 

 

I cannot tell from your ultra close pics what your biggest complaint is. Please note the skin will not glass smooth perfect after acne scar treatment, 50% after many treatments is considered good, if you get more great. your scars as such look pretty great compared to the ones I help daily.  

Some of those spots look like they could take some subcision and filler (your supposed to do up to 8 of these, instead you went the energy device route first which only softens the scars a bit), but hard to tell as acne scars make shadows and that gives me the best idea in a picture. The hair is also throwing me off. It basically looks like redness is your biggest issue with some slight textural issues. You did treatments in reverse, we never recommend doing laser first and so many of them, subcision must be done and tca cross work before moving onto energy devices. It's like working on a a pot hole befor it has been filled and leveled. Many Dr's just blast problems with energy devices. 

I see PIE or redness which is wounding post acne, this takes a year to go away. You can use vitamin C serum, silicone gel or cream, or the PCA Gel combo below. Vascular laser is the most effective on redness, and I Actually think this is your biggest issue in the picture provided, ... PIE/ reds make the scars stand out.

IF it's texture co2 laser would do nothing for this, or rf microneedling, as this is a textural issue. A better option would be a deep peel or fully ablaltive laser resurfacing, erbium. You can do tca peels at home if you wish and alternate months with something like a dermastamp aka derminator device. 

 

Use nightly: Retin-a or tretinorin(prescription), or Differin (Target/Walmart), The Ordinary Granactive Retinoid (if you can't get the above). Cureology(USA) & Yoderm(USA) provides online prescriptions with customized skin care creams for your concerns, those in UK Dermatica provides the same thing. Apply a moisturizer when your face is dripping wet. Wait 5 minutes for it to dry, and then apply the tretinoin. This cuts down on irritation, you can also skip days of application if it's irritating and you need to build up %. This provides skin turnover - new collagen. To prevent (reds & browns from all Dr-treatments to ethnic skin or to work on current skin discoloration), try PCA hydroquione free gel for Hyperpigmentaion (Amazon) or Cureology/Dermatica includes this in their products if ordered. Using this prevents hyperpigmentation from treatments. Hydroquinone has side effects for some people, natural option as listed above is better or ingredients kojic acid/Konjac and skin lightener ingredients, like Niacinamide by The Ordinary is great for pores and discoloration as well. The Ordinary, Cerave, Eucerin, Dr Sam Bunting Flawless Cleanser, and Acne.org's products are all great options for moisturizers and cleaners without fragrance / dyes / allergens. Sunscreen is a must (for the sensitive use zinc only physical ones)!

Thanks ,and sorry about posting question 2 times .

I have this redness from 4 years, after my acne cleared up. Here in India we don't have many good dermatologist ,I did what he suggested me.

i will follow Ur advise as u mentioned.

This is my progress after 4 sessions of spectra gold toning (used by dermatologist for redness) I didn't see any major difference, hardly 10 percent

@HuskyRayon Thank you for posting a picture, ... can you post a before picture to compare to. For some reason the above pictures are not showing up for me, I have contacted the admin. 

I would need to know the settings used, ... what was the response of the provider when you said you only received 10% improvement. You do know you need about 6 sessions with a pulse dye laser. If the settings are low depending on your ethnicity this will drag things out more, tell them you want to bruise and have stronger results and are aware of the down time. Do they have other laser's to address this if this does not work for you, a a good Dr does or has a IPL machine to also use. Again discuss with provider next steps. 

Are you nightly using a retinoid, niacinamide (the ordinary), and have you tried things like monthly glycolic peels?

Sadly I lost my smartphone and all my recent pics ,will try to find some old pics.

I forgot to ask about settings but he did 5 passes on me and my skin was red for 24 hours.

The dermatologist said i can't go more than 4 sessions, he said skin is not that red now (lord knows how can he say that)

They don't have other lasers,  he only had spectra gold toning handpiece .

My dermatologist said I will see more redness after using retinoid so he said no to retinoid. 

No peels ,the last mnrf he did was in Feb and after that 4 sessions of spectra gold toning (after requesting for vascular laser)

I don't  know what to do with this redness it's already  more than 5 years and this redness is not going. 

Should I change my dermatologist

@HuskyRayon Please consult with 3 other dermatologists, ... he is selling you on ""spectra gold."" This is common for someone who does not know what they are doing, has only one machine and is selling just that, is not a aesthetics dr but a general dermatologist, or does not want to take responsibility for your care. Sure a retinoid could cause redness, do it once a week, if you need mix it 50/50 with moisturizer. You don't have to do it nightly if you can't handle it. 

Anyone and I repeat anyone is red after a procedure, ... if your not either the procedure was not very effective, you heal super fast, or they are going low setting on you because they are afraid of sensitivity. I think It's the first and last on you. 

Your main complaint is redness correct, ... PIE currently? I can only go by the pictures you provide, if they are old I may be not up to date. That being said goto a Dr who cares about treating your PIE. This Dr does not seem good sadly. 

Low answer,... should you change yes. You need a cosmetic dermatologist, ... how do you know, they have a bunch of devices, their website, filler, they advertise procedures. General Derms don't care about these concerns.
