What types of scars do I have and what is the best treatment?

Hi 

I have been dealing with these acne scars for as long as I can remember. I™m 21 and never had severe acne, but I had mild to moderate when I was in high school. I™ve cleared it up but now these scars are awful and I feel like it™s the first thing people notice about me. I™ve tried 4 rounds of microneedling, then a year later (this past summer) I did 4 treatments of palomar laser. I got decent results with this but I am still left with unwanted scarring so I was wondering if anyone had any suggestions and knew of any doctors in the New England area

I don't think your scars would be noticeable to other people, they are so very very minor. Don't worry about them.

@Nursinggalx3Can you help me out. I don't see any scarring. I do see Rosecea or some redness and perhaps a pimple or two. Is this what your posting about PIE or redness or for large pores or something? Your skin is the mildest case I have advised in weeks so want to be sure. Sometimes less treatments are better as these can also mess up our skin if we do not have severe scarring. I think on your right cheek I see boxcars, but I am not sure without a closeup and shadows.

If not can you take a picture with a flashlight or directional light showing the shadows your scars make, sometimes we cannot see what you can without a picture like this.

These are at different angles and I think they show more of the extremity of my scarring. I believe that my skin just looks aged from it, too.

@Nursinggalx3I again looked at the pics you provided, thank you. I do not see any scars, are there tiny box cars?Your probably the most mild case I have ever seen. If you mean large pores Get 1927nm fraxel, several sessions, orange peel texture only get's a percentage of improvement. If you have slight box cars, at home or a dr do some tca peels and dermastamp any small surface box cars, and you can do paint on tca, some drs only do tca cross. Personally I would not do anything, treatments can cause more harm than good.

Would you recommend microneedling? And if so do you know what depth would be useful for my skin by chance

@Nursinggalx3Sure you can do this at home. Buy a derminator, you only need .75mm to 1mm monthly treatment, alternate with glycolic peels as needed. If you need a step up over many months 1927nm Fraxel several treatments or CO2 laser (optional).

Checkout @ 4 mins into the video 😉 , I can't state any more because of confidentiality agreement... That being said I am not your Dr and as such your should be under the care of a board certified licensed dermatologist for treatment, there are many paths to the same goal.

@beautifulambitionSo ageneral orange peel texture is difficult to improve upon? Would Clear and Brilliant offer the best chances for improvement? The improvements in before and afters pictures Ive seen,even coming from the C and B company itself, seem underwhelming. Perhaps there are particular settings that need to be used for orange peel texture in particular? Im in Toronto and its quite difficult to find a Derm or Medi-spa that are comfortable with using anything but the standard manufacturer suggested settings. Thank you for sharing your expertise so generously.

@nicky1818For Orange peel we do CO2 laser 3-7% density and the highest power, and 1927nm which is a fraxel product, in high density mode because it's a superficial laser, many passes are made. Clear and brillant can also be used, ... this is a Fraxel Product as well, just not as powerful, ... it's gentle for any skin type. More sessions are needed with this then standard 1927nm Fraxel. The setting is High Density, the Dr must over ride the machine, also many passes are done. 20mj for up to 20 passes in key areas.

Wow @beautifulambitionyoure the real deal! I follow dr lim on Instagram and my dream is to go to his clinic one day- thank you for your kind help. In regards to the derminator, I do not have a lot of confidence in myself using a machine on my face as I feel like I will make it worse, do you have any recommended brands forDerma rollers perhaps? I have yet to find one with promising reviews. Also do you have experience with using a dermaroller and if so how have they worked for you?

I know many here use the derminator, it's relatively cheap compared to treatments, ... all you need is settings between .5mm and .75mm, many go to deep with the needles and cause damage or over do the spot. Less is more. IF you want to go the cheap manual route, buy a dermastamp they sell them on owndoc or amazon,...IF your uncomfortable you can always try one treatment at a local med spa or Dr. Needling by itself won't fix things, it's in combo with acid peels, and tertinorin or differin usage. Then people kick it up a notch with Dr based treatments. Lim just did a video on Youtube on microneedling FYI ;-P I know Lim for Orange peel does fully ablative erbium in supper shallow cases or full density fraxel with some co2 work.

See Rullan in San Diego if you want to go the manual route with acid peels and cross. He is highly sought as well.

It's difficult locating scars in the photographs you posted. Your skin is in relatively good condition. Since many procedures are expensive or risky, I wouldn't recommend additional treatments. I'm not a dermatologist, however.
