Scar remodeling

Is it true that it takes a year for a scar to "remodel" and look how it will permanently ? I'm interested in fixing an atrophic scar on my chin. I also have a few hypertrophic scars on my chin as well. All my scars came from taking massive amounts of hormones for IVF (InVito Fertilization) I stopped taking all the hormones and no more active acne. So, my question essentially is , is it better to give the scar time to mature or act now? I'm not too far from Dr . Novick and after stalking this page , he seems well respected. I should also add that I'm taking spironolactone because as I said all my cysts were hormonal and in chin area.

1) Actually, it might take longer than 1 year. More conservative estimate is between 18 to 36 months. The reason is that collagen production takes time, especially if you're older. It takes up to 6 months for full collagen rejuvenation. Now, you usually need at minimum 3 treatments to see optimal result. Hence 3 x 6 = 18 months. But the problem with scar resurfacing is that you usually need to repave the road after filling the pothole. So once you build enough collagen, you will likely need to resurface any rough scar edges. This will require additional time. Hence, scar remodeling requires two separate subsets of treatments, one under the skin (subcutaneous) and one above, that can take up to 3 years.

2) Unless it's a "fresh" wound, you're better off treating the scars more sooner than later. However, once the treatment kicks in, you'll need to allow the collagen enough time to grow, as already mentioned above.

Thank you for the information. These are the scars I'm dealing with. As you can see my chin is totally uneven and the long vertical scar is  the one I hate the most. What should my plan of attack be? Also, should I even bother with local derms or go directly to a specialist in field. I'm in upstate ny so it looks like Novick or i think Rappaport in New Jersey . Also, I know I have some texture issues on my cheek, but my chin is what is of most concern .I've also lost a significant amount of weight in  short time which made everything look even worse. Plus, I'm in the early forties so the plumpness in your face was already depleting... ugh!IMG_7240.thumb.JPG.3fbee46b17fd7617298e1a685c1407f4.JPG

I see one indented mark on the chin, this can be fixed by filler from any expert injector. I think Novick was be a great choice for fillers or Rapaport, both are good, ... Novick is very good with fillers. Rapaport is a general derm who has the laser etc... I see you needing volume in the upper cheek tp pull things up (age related fat loss) and soften the smile lines. For a case like yours Rapaport would use some laser or Infini rf microneedling and Novick can do a Deep peels. I don't see texture issues on your cheek, just aging related skin. Perhaps consult with both Drs and pick the one who feels right for you. Plumpness in the face and the indent in the chin is all fixed with fillers.

Inquire with Novick or others about Kenalog or 5-FU.

Thank you both for the help and recommendations. One final question . I realize Ha filler is temporary and that is fine. I would never use something permanent like bellafill, silicon etc. But, does HA filler still stimulate collagen? Basically, I'm asking is it possible that after a while ( years) the dent might fill in itself from the filler stimulation that I would probably top off every 6 months to a year? Also could subcision help? I know it's mainly used for deep tethered scars . Here's another photo from a different angle.

If you like do sculptra, at your age there is fat loss and bone loss so this would help overall in the cheeks, it's a bio simulator to make new collagen. Laser or a peel would also help.

For the dent it's one area so you could do HA and hope tissues develop below (does for some and the time between injection increases with time), ... yes you can subcise that area, ... eventually you can get something more permanent if you wish (though you said you'd never do that) or scar revision where they suture the area and laser it. I would not start here though (only do that with a plastic) - filler and sub should be fine.

Consider pairing Sculptra with Infini (or similar RF microneedling).

Be sure to get Sculptra at least 4 weeks prior to Infini. One Sculptra injection per RF suffices, as oppose to the guideline of 3 if you were getting the filler alone.
