Best Treatment option for my acne scars? Pics inside

Hello all, I am mid twenties male who has had acne since about 16. It has been at times moderate, and at others I guess persitently mild. I used the regimen in the past with great success. Past year haven™t had to use regimen as just had occasional spot here and there, so put up with it. I just do cetaphil cleanser in shower, cetaphil moisturizer after, or spf neutropenia facial moisturizer if going outside now a days. 

My scars have become more apparent recently. Luckily I only have gotten acne in my cheeks, myforehead and temples were spared. But I have tons of what I believe to be ice pick scars in cheeks, also large/blown out pores etc. Also lucky for me I am having a bit of a breakout at the moment on top of this. I realize I have facial hair in pics..but my hair unfortunately doesn™t hide my scars so shouldn™t be an issue. Just wondering what my best course of treatment would be for this type of scarring? I am amazed by Dr. Lim, but don™t have money to go to Australia now. Could see derm in USA, particularly in NYC as I am in northeast. All these scars have been here for years, so I assume they™re here to stay. I wonder if micro needling would help them? My scars still look bad when i stretch skin...so whatever that indicates count me in. 

I tried to take pics under brutal lighting and some up close and angled with my iPhone 7 Plus camera ...and the actual camera with more pixels not the weaker inside one. Damn the look brutal lol. Thanks again guys for any input...I really appreciate it!

Well, I don't think anyone would would notice unless you're under harsh light as in these pics. If it bothers you that much, I suggest you try a few rounds of TCA peel for starters. Check out the FAQ Section [Link removed] for more info.

Looks like icepicks. You can probably TCA Cross them, though they will become slightly wider as they become shallower (at least mine did), so you might want to consider that first, as they are not very noticeable.

Please get checked out for Folliculitus as this is what it looks like to me, infection at the hair follicle. Yes TCA Cross (or fraxel 1924 at full density/ Clear and Brilliant is similar just more treatments ) would work best for your icepicks but you need to be clear first or you could have a bacteria problem. I really do not see a problem with your skin other than pores / icepicks. Good skin quality, not fat loss. You could do a few peels for texture.

Please be completely shaven before any procedure.

As soon as i see your beard in pics i think folliculitus also,scrolls down and BA is mirroring my thoughts,suggest drs and course of anti biotics,its best thing to kill it.

looks like you will have to shave as i suspect your scarred pore prob in also under the beard area.

suggest you get anti biotic,oxytetracycline (or similar) get clear,shave and do tca peeling,follow the tca peeling guide lines.

needling is more for deficiency where collagen production is required,everything you got is right at the surface.

Thanks for input everyone. I never thought of Folliculitis honestly...as Ive definitely had acne for years so just always thought every pimple/blemish was acne related. I will try to get into doctor to get checked out for that, and see if I can get some antibiotics. Im gonna shave tonight and upload some pics so you can see scars better unobstructed from beard. But most/all of my scarring is unfortately in upper cheeks where my facial hair cant really hide them:( Back to the Folliculitis talk...could this be causes by yeast overgrowth in body? Or is that all just bs? Thanks again

You can get a biopsy if you wish. Yes one can get candida, yeast/ fungal infection, etc... from Taking too many antibiotics, poor health, stress, gut issues. You can try the basics for a week and see if they help, no sugar, tomatoes / eggplant can cause some issues, dairy including cheese are triggers for some.

Fungal acne is different than normal acne though.

I don't think it's folliculitis, as you can clearly see acne comedones in your pictures, which are not present in folliculitis.

Folliculitus will basicly just manifest itself as acne on the face.

 This is not true. Acne and folliculitis have different morphologies although distinguishing the two can be tricky.
Folliculitis will be pustular and there will not be any closed or opens comedones and no nodules and papules. This is a good example of folliculitis:

@CLOODWhat are your qualifications to make this determination, are you his derm, are you a derm, or did you do a biopsy on him?

Oh Look I can paste pictures too of Folliculitus, infection of the hair follicle. OP don't listen to me if you want, goto the doctor and discuss. Armchair specialists who know nothing will not help your situation of infection in your hair follicles. Hence get a swabbing / biopsy done.

  • Reaction toDemodexmite infestation may occasionally be misdiagnosed as folliculitis.

  • Oil folliculitis is inflammation of hair follicles due to exposure to various oils and typically occurs on forearms or thighs. It is common in refinery workers, road workers, mechanics, and sheep shearers. Even makeup may cause it.

Treatment[edit]

  1. Topicalantiseptictreatment is adequate for most cases
  2. Topicalantibiotics, such asmupirocinorNeomycin/polymyxin B/bacitracinointment may be prescribed. Oral antibiotics may also be used. Could needprobiotictreatment.
  3. Some patients may benefit from systemicnarrow-spectrum penicillinase-resistant penicillins(such asdicloxacillinin US, orflucloxacillinin UK)
  4. Fungal folliculitis can worsen with antibiotics and may require an oral antifungal such asFluconazole. Topical antifungals such as Econazole Nitrate may also be effective.

Folliculitis may recur even after symptoms have gone away.

There is no need for this condescending tone. I was just giving myopinion based on the pictures. I just finished my dermatology rotation in medical school and our professor just made the point about the absenceof inflamed comedones in folliculitis. But anyway, go to the doctor and get a bacterial swap done. It can never hurt.

Do me a favor, see your derm and get confirmation swab/culture but also trynizoral shampoo (pretty cheap) use as face wash and see if it helps (must sit on for several minutes) do a google search for "fungal acne". You mainly have icepics and need tca cross and a tca peel for the textural bumps. Do not peel till you clear papuals.

Ok I will try the Nizoral...I had read it can eliminate yeast based folliculitis. If bacterial would just BP work? Of course Im gonna see what the doctor says as well. Thanks
