My 2017 Subcision + Filler Experience/Results

Hello everyone, I decided to go with a combination of subcision and filler (Juvederm) thanks to the great knowledge provided by "beautifulambition." This forum a whole lot better place with him here, and I appreciate him for taking the time to help the people on this forum.

After doing careful research, I ended up choosing Dr. Peter Rullan due to the great experience some other people on this forum had with him, and he was recommended by beautifulambition. He's a fantastic, caring doctor and I highly recommend him. Besides having a superb doctor on your side I think it's also important to take care of your health as well, so your body can produce the collagen needed.

Below are the pictures before the subcision. I've not had any prior treatment for my acne scars, which are between 1-4 years old.

The procedure took about 30 minutes. Getting the anesthesia, surprisingly, wasn't painful at all; I remember it being a bit more painful for dental work a few years ago. I didn't have any numbing cream either. Overall, I'd give the whole procedure a 3/10 for pain. Immediately after though I had some wicked swelling, and I did start to feel a bit more pain. The pain was went up to 5.5/10 right after the operation. I didn't take any pain meds. I just took some vitamin c and protein (MacroBar snack bars from Whole Foods); shortly after I took a 2 hour nap. By the time I woke up the pain was gone. I ate a lot of salads, green smoothies, and protein (chicken, 100% grass fed beef), so I can get my body everything it needs to heal properly.

3 Days After --

I decided to take a shower about 24 hours after the procedure. I used a gentle cleanser for my face as well. I then applied a moisturizer with vitamin c. I applied the cleanser and moisturizer each day once per day. After 2-3 days the swelling went down big time. I didn't apply any ice since I was told the more swelling/bruising the better the results.

The yellow swelling started on the 3rd day, and lasted until the 6th day. My eyes got slightly affected (you can see the above pics), but it quickly faded away.

5 Days After --

I had a follow up for more filler. The doctor said everything was looking very good, and he did not think I needed much more filler. He used a little over half a syringe in total (first day + follow up) I believe. The doctor noticed a spot he missed for the subcision, and he treated that as well. He also noticed a raised scar right next to my left-side's laugh line, and he offered to treat that as well for free.

The possible final improvement looks very promising. It's hard to tell, for me at least, what the final improvement will be since some areas are still slightly raised up (swelling/bruising, etc). But each day it's getting better and better. My favorite part of the day now is sleeping, since that's when you heal the most :) Will follow up about 5 days from now.

Cool man, In a very similar position and am looking at both subcision and fillers for my temple area. I have a few moderate boxcar scars on each side. Would really love to see your progression and the type of results that you end up with. Would love to see you update us along the way.

The bumps are still there, but they're getting better each day. The left side being a lot worse than the right.I've started applying Manuka Honey as a mask yesterday to see if that would help speed up the healing. The improvement I could end up receiving is easily over 50% I reckon, but it's hard to really know until the bumps subside. I will be updating with more pictures soon.

Thank you for sharing. My first sub/filler is next Friday. These pictures give me a good idea of what to expect with bruising and swelling.

Will follow your progress and root for you.

Dude those results look amazing so far. I'd say you may very well easily end up with over 50%. I'd love for you to keep updating and i'll check back every week or so. How long did the doctor say the juvederm will last? Did the doctor give you an idea of what percentage of results you will end up with? If you end up with those results I might be taking a trip out to California pretty soon.

@Guardian10 Congratulations and i agree with you, Dr Rullan is awesome. I flew to get my subcision done with him. I didnt get the filler done, just subcision and i did the suctioning for 2 weeks.
I was wondering how much he charged for subcision + Juvederm?
To me, he recommended either Bellafill or Sculptra, not sure why he did juvederm on you?

QuanHenry -- No problem. I will be posting an update on Wednesday. I hope your subcision goes smoothly, and you get great results as well.

thepwhisp -- Thank you. I will definitely be posting frequent updates. The doctor said the Juvederm should last for around 6 months. I never asked what the percentage of improvement I should receive. He said the results should be really good during the follow up, though.

keving100 -- I flew to see him as well. His assistant mentioned that people all over the USA fly for his subcision treatment, and said one even came from Canada. I told him I didn't want anything permanent, and we ended up on Juvederm.

Minor improvements to the bumps; slow and steady. Does anyone know what those white circular bumps are on my left side? They've been there since the swelling went down after the procedure.

Oh, I was actually referring to the bumps; and not the actual scars. The scars on the areas that didn't experience bumps I'm extremely happy with. I just hope these bumps don't linger on for months, because someof them are quite big. I've attached new pics:

I bought a Clarisonic vibrator for the skin. I plan on using it on the affected areas twice a day. If there's no improvement then I'll probably get a saline injection to decrease the size.

I saw that you want to do next time a volbella filler. Does it mean that you don't like the juvederm?

@Noa27Volbella is Juvederm, it's just a type that does not attract water and make bumps on shallow skin. Voluma is more if you are deep in the cheek like Quans and want plumpness. This does not work int he temple or forehead / eye area. So ask if you have shallow surface scars for a non water attracting filler. This is the same thing they would put under the eyes normally.

Thanks BA :) Next week i'm scheduled for hopefully subcision with filler :)

 So i'm checking some fillers out. I also read stuff about fillers with biostimulators. So maybe i'll go for radiesse. 

I don't want to hijack a topic, so on topic :D

is Juvederm Volbella similar to Juvederm Ultra? I had Juvederm Ultra couple years back right into my acne scars, but the filler got dissolved after a yr or so

Juvederm Ultra caused lumps for me (easily fixable -- HA fillers are reversible unlike BellaFill), so I'm going to stay away from Ultra from now. I didn't pick the filler, however. I just told the doctor what's the best filler for my scars that's not permanent (BellaFill). I should of done more research on the different types of HA fillers beforehand.

I went to see a dermatologist today. She recommended I wait another 3 weeks and just massage the areas with a warm washcloth. We did end up doing a Saline injection on some of the worse spots to dissolve some of the filler. Picture below.

@Noa27you don't want radiess, its not a filler, it sets on bone to define it. Aka the bone under the eye, and the chin. You cannot dissolve, it's not used for surface acne scars or subcision,

@keving100It's between Restalyne and Juvederm Perlane / Lyft, does not have the longevity of voluma. Hydrophilic (Water attracting). Popular in Europe. In Canada you have things we don't have in the USA as well.

@Guardian10I know we discussed this but for information exchange. Vobella is better for boney regions, it does not attract water, what you see is what you get injection wise, perfect for under eyes and surface acne scars. Water attracting fillers are great, if you need volume, not if you don't want things to possibly change when you leave the office from water attraction in boney or surface areas.

I ended up getting the Hyaluronidaseinjection yesterday to dissolve the filler. I went to a plastic surgeon's medical spa. He has fantastic nurses that have lots of experience with fillers as they do it pretty much every day. I saw one of the nurses and ended up paying $100 for the injection. After 24 hours the lumps have gone down big time, so I'm very happy with that.

I'm going back tomorrow as there a few spots that need just a little bit more of the hyaluronidase. After that I'm I'm getting microneedling + rf at the same place. The machine is called Vivace, which is very similar to Infini rf. I'm getting it to help with some very minor superficial scarring on temples that Dr Rullan missed, and also for better skin texture and even skin tone.

I've noticed no increase in acne scars as the filler dissolved btw. Most of the lumps were in places that had very minimal or no scarring. If I had to it all over again I wouldn't get filler at the same time as subcision. But would wait for the swelling to go down, and then visit a expert injector that does filler all the time.

My only concern is that the heat from vivace might negativelyaffect the duration of the HA filler. Have you talked to the doc about it?

Subcision which is not typical hypo subcision as there is more swelling. Many here do not have the down time, and are visiting somewhere, hence they want it all at one time. Always best to separate out procedures. IF it's about the money pay as a package price and go four

I got most of the filler dissolved, and didn't notice a difference in the scars. Most of the filler seemed to have been applied in non-acne scar areas.

The nurse did mention the Vivance can do in-sulated and non-insulated. But they temporarily only have one at the moment, but the other one is coming soon. I can't recall which they have now; in-sulated or non-sulated. Anyways, appointment scheduled for Monday. I just got the 2nd round of hyaluronidase at no charge as well.
