why does acne get worse before it gets better with accutane?

does anyone know why acne can potentially get worse before it improves while on accutane? do you have to have a certain amount in your system before it really starts to work?

i've only been on it for about a week now...experiencing what seem to be normal side effects (dry skin, chapped lips..), but the existing acne is more painful and my face just kind of hurts. is that normal??

Hi. Accutane does take some time to "kick in", this is because the results of it's action take time to see (with the turnover of your skin cells). Accutane (being a retinoid drug) causes skin cells to grow quickly, and this sudden abundance of skin cells can get trapped in the pores causing comedones (ie whiteheads and blackheads) to form. At the same time, the drug is working to reduce the oil production in your skin. Eventually, your oil will reduce and the anti-inflammatory effects of accutane will start to dominate and you'll clear up. I can't tell you how long it's going to take, but it's probably related to how severe your acne is to start with. I would say that the more gunk you have sitting in your pores to start with, the worse your IB will be :(
