if your derm felt like it's time to end your course, but you want 1 more month, how do you convince your derm to give you one more month?

a lot of derms seem to have big egos, at least mine does

I anticipate she'll probably end my course one month earlier than I would like, and I want a higher cumulative dose to reduce the likelihood of a relapse

tell her exactly that.

Do the math, tell her what your cumulative dosage is. Tell her you want to hit close to 150mg/kg. If your blood tests are fine, I don't see why she would disagree. Don't go in hostile, but just show her you know what you're talking about. If you've hit 150mg/kg and are still breaking out, she should give you another month.
