Source of the deep pimple?

Do the really deep pimples (that never really surface and then take 7+ days to go away) get caused by the same thing that surface pimples do? I went to a new derm who told me that all acne is going to occur where there are hair follicles. When I showed him my problem area (parts of my face where I don't shave), he said that there are indeed very tiny hairs there.

What doesn't make sense to me is this: How can such a tiny hair (not visible to the naked eye) cause such a massive plug? It just seems that the deeper pimples have a different cause.

You have tiny ('vellous') hairs, over every square inch of your body, except your soles and palms.

A spot does not occure 'because' of a hair, but rather, will occur due to the follicle itself, which also happens to be where the hair grows.
