Cumulative Accutane Dose?

Hey everyone,

I did a course of accutane about 2-3 years ago and while it cleared me up at the time, I eventually relapsed. After that, I looked into alternative treatments, particularly Dan's Regimen with BP, which after 4 or 5 months managed to do a good job of clearing me up, but then I shortly thereafter relapsed yet again despite maintaining the regimen.

After slightly over a year on Dan's Regimen, I decided to give it up. While it had improved my acne slightly (and significantly in the past), I didn't like the sensation it left on my skin. It constantly tingled and felt dry and too tight, even though I regularly applied moisturiser in complete accordance with the regimen.

So I gave it up, somewhat cold turkey, which was a bad idea in hindsight as I was bombarded with a cluster of pimples particularly around my mouth. I had never experienced them in such high concentration -- it was to the point that it was painful whenever I opened my mouth. But after a month or two, my skin seemed to have calmed down from the BP deprivation.

I decided to give accutane another shot after learning that second courses were not uncommon.

I've been taking accutane for almost a month and a half now. So far I've noticed a slight improvement but nothing to write home about, which is fine as I wasn't expecting anything much yet.

However, I'm trying to be extra cautious about my cumulative dosage as I want to know that I tried my best to minimise the chances of a relapse.

I'm taking 60 mg/day for just over 4 months, which will give me a cumulative dose of roughly 115 mg per kg of my body weight.

Now, I learned that the range of the cumulative dose they recommend is 100-120 mg/kg, which means my 115 mg/kg sits comfortably within that. However, there are other sources that state the range is 120-150 mg/kg, which I sit just outside of.

So my question is...which range is it? Should I consider requesting to up my dosage, or would my 115 mg/kg suffice in terms of staving off a relapse?

Any help would be much appreciated!

i think its the 120-150 range. for me, the higher the better. i actually reached 200mg/kg and my skin looks beautiful and havent had any long term effects. it couldnt hurt to bump up your dose for the last month if your derm agrees. good luck

Really? Because the accutane description on this very website specifies the 100-120 range.

I just find it strange that the two ranges don't even overlap -- the lower one's maximum is the higher one's minimum.

Anyway, I suppose I could finagle an increase in my dosage for the last month if that's the case. I just wish the information on this wasn't conflicting like that.

I think the range is between 120-150mg. However I think a difference of 5mg or a little more won't make such a difference. Every individual's course varies so you can add or take a month away. To tell you the truth, I didn't even bother calculating, I just left it in the hands of my derm to decide how long to take and the final cumlative dose.

I'm sure your derm has calculated out for you so not to worry.

In case you wanna know, I was on 20mg/day for 8 months and I weigh 45kg. I didn't calculate mine as I said above so I don't know how much I have taken. So far I'm over a year post-accutane with no relapses.

My derm never gave a specific number but said that she really bases it more on how a person is responding to it then how much they are "supposed to get".

With that said my estimated cumulative dose will be between 160-180mg (most likely 180mg) even though I do not have that severe of acne.

wikipedia says "Efficacy appears to be related to the cumulative dose of isotretinoin taken, with a total cumulative dose of 120–150 mg/kg used as a guideline.[9][10] High dose treatments should only be used as a last resort due to adverse side effects."
