Most people do not get a serious flare (IB).

Of those that do, it is generally coorelated with starting with a relatively high dose instead of easing into it and having acne that is under poor control when Accutane is started. So people who have been using a topical retinoid or a systemic antibiotic for awhile before starting are less likely to experience a flare. If you are really worried you can ask your derm to prescribe a non-cyclin antibiotic to overlap the beginning of the course to minimize any flare.
