It's always been somewhere in the more severe end of the "mild/moderate" category, no cysts thankfully, but still enough the grant me a somewhat clippling social anxiety. Let's just say my romantic life and self confidence hasn't been the best, partly because of this.

Yeah, you get it, and a lot more that i don't remember too of course, we're talking 10 years of trial and error here.
What I've been on these last 5 years or so is what worked best for me:
Benzoyl Peroxide 10% and a moisturizer, daily before bed.
And Don't eat any sugar, no joke, this is by far my most effective method of prevention.
But that shit fucked up my skin pretty bad and now it's almost constantly red and I blush easily.
Coupled with it being thin and almost wrinkly.
Yeah... and still having acne too.
I live in Sweden, I am naturally pale as snow, and the redness shines through like an unwanted evening sunset.
I had enough and talked to my dermatologist.
He immediately offered to prescribe Isotretionin, concidering the scars on my back and the persistance of the problem.
I said: let's do it.
