So, it's been a while since I've posted. I started first month 80mg per day. Then the next three months I went up to 100mg per day. Now this month I am at 120mg per day, and will do that next month as well. After this month I will be near my cumulative dose for 1mg/kg body weight. Instead of taking my last month at a lower dose and still hitting my goal cumulative dose, I am going to go aggressive at it and do another month of 120mg/day. It's fine, some people do 1.5mg/kg of weight or even 2mg/kg body weight. All depends on how bad acne is. Mine isn't horrible, but it is my 3rd (and want it to be my LAST) course. My first two courses back in 2010 and 2011 I only ever took 80mg a day due to insurance. I most likely never hit my cumulative dose and never took enough per day for my weight of 230 lbs. As these months have gone by, besides the bloody noses and skin scales dryness, the only real thing I have noticed that is a side effect is my workout's and lifting have hit the dumps. I get off the ground from stretching or doing something on the floor like I'm 70 years old. Low back and mid back pain when sitting, and when doing sit ups. General fatigue and strength loss. I can lift 50% of the weight I could before I started. My cardio hasn't been effected much. Wrist tendonitis and pain from doing push-ups, now it's nagging. So general bone/joint and ligaments ache. I cannot wait for this to be over in April so I can watch my body rejuvenate and come back to fitness and strength. I don't even care if I get pimples again, I'm not going on this again. With how mild my acne was this may have been overkill but my acne kind of bothered me. But, albeit, as an Exercise Physiologist - in my job and in my life - I am an active person and have to watch my weight and physique so this workout stamina thing is really killing me. Okay, so started day 15 today. My lips are definitely dry and my eyes as well. I have to put eye drops in my eyes at least once a day. This isn't helped by the upstate new york cold weather and the heat being on in the car and house. People who live in humid areas may fair better than me in this regards. I use nasal spray and aquaphor on my lips when I'm at home at night. I am noticing less pimple forming now and the pores actually looking smaller as well. My skin is making much less oil. If I don't apply my Simple™ facial lotion after I shower and shave my face, my face gets very flaky near the areas of a lot of movement like my mustache area, lips, sides of my mouth, and chin. Cheeks, and forehead are not flaky (yet) but they are a lot less oily throughout the day.
Ok so started day 8 today I believe. Today 100% notice a difference in my oil production; that it being way down. I am so oily and always shiny to the point where people ask if I'm sweating. I can wipe my head at work every 15 minutes and I will look oily again. Today its been 7 hours already and my face still looks matte and not shiny. I am going to have to start using facial moisturizer now. I started chapstick and nasal spray already. My hands and knuckles are dry; it doesn't help that I work in the medical field and wash my hands like 20 times a day while at work either. I'll keep you up to date shortly. Have a couple cystic pimples that were big and came to a head in 2 days around day 5-6. The remnants of them are still healing, and one may be active or new on my right cheek. I wouldn't exactly call it an initial breakout, but maybe that was it? They were most likely cysts already in the dermal layer that were dormant being aggravated and cycled out.
Okay, so started day 5. Been taking my Absorica at night with glass of whole milk, a spoon of peanut butter, 3 fish oil capsules, and 8 pork rinds. I have noticed my strength and energy have gone down a bit. I was working out like crazy before I started and now I just cannot keep up with where I was. It's okay though, I'll do what I can, it's better than sitting stationary and doing nothing. Definite nasal dryness now, and definite dryness post shower. I go to work every day with eye drops and chapstick. I shaved today the bottom of my neck to keep my beard trimmed and also the back of my neck where my hair grows past my hair line, and it is irritated now even with lotion. Feels a lot like before when I did my first course or 2nd course - so I know it's working well, I'd say another week or two and I'll be really dry. I am doing 80mg a day, going to 100mg a day next month.
Day 3 started today with my 5th dose. I noticed after last nights shower that my skin felt a littler tighter and dry than usual. Some definite nasal dryness starting today. Facial acne looks the same though. *One other thing about Absorica; though it does absorb like 83% better on an empty stomach than regular generic Accutane, it still benefits to take any of these with meals with fat. So since I have the Absorica I don't over do it, but in the morning I make 3 eggs, and take my pill with a glass of whole milk. At night I eat dinner, wait about 15 min and then take my pill after eating some almonds, a spoon of peanut butter and some whole milk. Any fat will help this medication absorb better - ESPECIALLY regular generic Accutane such as Amnesteem, Sotret, and Claravis. These don't have the Lidose technology that Absorica does so these especially need meals with high fat contents.
A little background: So I did one course of Accutane, the first using generic Amnesteem. I relapsed with acne shortly after stopping, and went on my 2nd course maybe 4 months after my first using generic Sotret. My insurance didn't allow for more than 80mg a day, so that is what I had to deal with even though it was under my weight for 1mg/kg of body weight. I am a 28 year old, athletic male, 240lbs now, during my first two courses I was around 225 lbs. I took my 2nd course for 7-8 months to try and reach my cumulative dose. Well, now 2-3 years after my 2nd course - dun dun dunnn.. the acne is coming back. Mainly facial. The chest and back/shoulders have always been better overall than before with little to no acne anymore. My facial oil is coming back hard and I'm starting to get the typical cystic acne from before. I ordered the acne.org regimen but thought to myself between getting up for work at 5:45am and getting to the gym after work at 7:30pm, I don't think it will be easy for me to take 15+ minutes to make sure I do the full washing and treatment regimen with the acne.org products. So, I made the decision to give Accutane one more go. I went to the derm, and he said since my last treatment 2-3 years ago, there has been some new studies showing how important diet is during Accutane for it to absorb properly. High fat meals, makes sense, since Accutane is a synthetic vitamin A which is fat soluble. But sometimes I took it with a small breakfast, or a lean dinner like chicken and veggies to accommodate my athletic eating routine, so I don't think I got as much fat as needed, so the derm thinks that is one of the reasons I relapsed or failed my first two treatments. He told me about Absorica, and how it is already in a fat solution in the capsule which makes it absorb like 80% better on an empty stomach than regular generic Accutane. I was sold. It still probably benefits to take Absorica with fat so I have eggs in the morning and take it with glass of whole milk. Regardless, the drug needed pre-authorization from my insurance company because it is brand name and there is no generic's for it yet. My derm got it to approve by stating that I failed my first two treatments with regular generic Accutane (amnesteem, sotret). **So my 3rd journey begins. I had no side effects from first two treatments except the typical extreme dryness. I've already bought my moisture products which I've found to work best: Simple Brand facial moisturizer Systane 'Long Lasting' eye drops Blistex Lip moisturizer sticks (3 pack) Eucerin Aquaphor Ointment (41% petroleum) [use for extreme chapped areas like lips when I'm at home before bed, and inside nose] Saline nasal spray My girlfriend makes me a coconut oil based lotion for my arms and body if it's needed in areas Took my third dose (Day 2) this morning, and I'm not sure if it's the heat in my car or the winter air starting but my nasal passage already seems a bit dry today. Any questions, please post. I'll update during the first month when needed, but I don't see a point after that. My acne isn't very bad, it looks typical, but I'm 28 years old and this is "adult" acne. It bothers me. If I was 19, okay so be it. But working in the professional world and dating, I feel like an adolescent with this acne so I wanted to blast it one last time with isotretinoin! It's hard going from completely clear during other treatments and then having your acne come back. It's like if you were fat and lost weight and became ripped for months then one morning you woke up completely out of shape and heavy again. Once you see the perfection Accutane causes, you need to have it permanently. Just my opinion.
