It sounds like you may have hormonal acne as most women do.  I am on Spiro and was recently on Spiro + Yas.  I would throw a hissy fit.  My derm was the first person to prescribe this to me, but she really had no experience.  My endocrinologist now prescibes it for me as it is a treatment for PCOS.  Perhaps you can try that route?  I feel for you and hope you can find a solution.  There may be another physician in the area that would be open if you bring your current Rx, perhaps have your Dr in San Fran write a note?  It is off label use in the US as well, but more mainstream for PCOS sufferers.  I wish you the best.

Just wanted to sign in and share with everyone how appreciative I am for this board and helping me in achieving clear skin.  It was a lifelong battle and finally was diagnosed with PCOS and started Spironolactone and Ocella (generic Yasmin) last year, Dec 30th to be exact.  What followed were 4+ months of worsening skin, depression, anxiety and I almost gave up.  Then the medicine finally started working.


Medicine may not be right for everyone, but I can say that it changed my life.  I dreamed of clear skin, not worrying about my skin, living a carefree life.  It took 33 years, but I was able to find this.  To all the ladies that have hormonal concerns and considering, I am happy to share my experience and urge you to continue to look into testing- hormones, endocrinologist, etc.  I continue to be thankful and pray on a daily and weekly basis for all the support and information you all provided me.  I am feeling thankful and sending love to all of you on here.

I've been taking it now for about a month and a half. I feel like my breaking out has decreased, I broke out a bit more last week just because it was that time of the month, but nothing cystic. I get a a small pimple now on average maybe once every other day. I actually lost 5 pounds so far, but my bust has increased in size I think. does this sound normal?

Yup!  All pretty normal.  It took me about 3 1/2 months to finally break through but my dose was low.  I did gain weight after upping it to 75mg with BC.  My boobs did grow a cup size plus.

Hey there!  Just thought I'd share my experience.  I started on Spiro 50mg  and Yasmin on 12/30 last year.  It took about 4 months to stop the constant breaking out.  I had mild hormonal acne prior due to PCOS.  It was only after I upped the Spiro to 75 mg with Yasmin on the 4th month that the oil production slowed down and consistent clarity began.  I did gain a little bit of weight due to the estrogen increase, I also grew at least a cup size maybe 2.  I just have to watch my diet and may go on Metformin to balance my insulin, issue with PCOS.  How are things going for you?

It's been a bit since I've posted, so thought I would come back and share.  My skin has really been a gift.  Thank goodness is turned around the way it did as I had a complete upheaval in my life, recently laid off and had to start interviewing, got a contract job right away and starting a permanent one next week.  Having clear skin and confidence back for that, I can't even begin to thank the Lord for listening to all of my prayers. 


My oil production has been completely reduced, I have a healthy glow and that is it.  I can go for hours without any real increase, just dewy at the end of the day.  I still am using the mandelic peels to keep my pores clear, it helps with just getting rid of dry or any potential clogged pore.  If I even have a clogged pore, it's easily extracted and nothing comes of it.  Did have one bump on my hairline last week, from getting my haircut and the product that the use.  I didn't wash my hair for two days, so expected something, but that was it. 


Weaning off of doxy, taking 1 less every other day.  Taking oregano oil on those days to try and also help my tummy bounce back.  I will then try 1 less for two days in a row, etc then 3 days in a row until I am at 1 a day and then every other day.  I just don't want tummy or other rebound issues so would rather take sweet time to slowly wean.  Thankful beyond words.  Have been freer with my diet as well and so far so good.  Spiro is truly truly been a miracle.  It was worth the torture of those 3months and appreciate all of the support I continued to receive.  Love to you all on your journey!!

Funny how I use to count the days and now I don't even pay attention.  I have been reflecting a lot on the journey that I have been through the last 4 months and I feel like so much of my life and time was wasted caring so much and being so miserable.  It was something I had to go through to get to where I am now.  I am content, my face is pretty much clear.  I only have minor tiny clogged pores every once and I while I either clear with a peel or exfoliate away.  I have had consistent clear face since after upping to 75mg Spiro and that small breakout.  I am going to skip my period this month and take my next pack and no actives.  The progress is great, I want to keep it up.  I have noticed my cup size has increased dramatically, they are super sore and I have gained a few lbs.  It may be due to changing my eating habits a bit, but also attribute it to the new level of hormones.  In 1 week I am going to start weaning off doxy slowly until completely off.  Probably will do it over 6 weeks.  I will document progress here.  I have been using peels on a weekly basis to keep exfoliation and even out skin tone.  I have tons of freckles and a bit of melsama from the sun, so would be nice if that faded a bit too.  I don't wear makeup, just a few dabs of concealer on my eyes and a few red areas that are taking a looooong time to fade.  I have lactic 55% that I may try next or the fade peel.  Overall my self esteem is slowly healing and I am trusting that I am on a great treatment that is working.  I was so afraid for so long, I am not letting fear take hold any longer.  Enjoying all life has and looking forward to the continued clearing and my skin getting better and better!

Hang in there!!! I have been there wayyyy too many times before and know how painful of a place it is to be.  I did always feel better going in to work and not letting acne stop me fully.  No matter how many people tell you that no one notices, it can be so hard.  But they don't notice.  I had a terrible breakout from starting Spiro and BCP and left me in your situation for a few months, wishing days, weeks and months by.  You will get through this, you will heal, life will get better.  Focus on just getting through today, baby steps.  Breathe, stay away from mirrors.  Dress up a bit if you can, see if that helps you feel any better about yourself.  Sending you lots of positive thoughts and hugs today!

If this helps you at all, I only started having a flare up last fall around October/November.  My skin had been fine before that, maybe 1 or 2 small little spots every few weeks.  I didn't worry about my skin at all.  Then my hormones went all wacky and they weren't getting any better so I tested my hormones and knowing the issue helped.  I forget how old you said you were, but I am 32 and experienced a similar flare up when I was 26/27 out of the blue my hormones acted up.  So, if you think that Spiro is an option you may want to look into the testing of your hormones.  Only you know what is best for you when it comes to how long you can wait it out.  Acne is a symptom, now finding out what it is a symptom of is the really tough part for some.  Mine was easy...PCOS...testosterone and there is an easy answer.  You are determined and focused, so don't stop until you get the answer and the treatment that you know will help you get back to where you desperately want to be.  You will heal, the scars will fade and this will become the past soon enough.  Stay strong, but don't give up on finding a solution that will work long term for you.

Yeah I dont know what to do. Granted my acne is still mild knock on wood. Im not getting cycsts thus far as most other girls have experienced but again prior to this I didnt have acne prone skin sad.png. Its mild but persistent and my face is turning into one big old scar. Im thinking about trying out dong quai. But your absolutly right I dont know exactly where my problem is. To my understanding Im not showing signs of estrogen dominance. A part of me wants to wait this out and another part wants to just get rid of this now with some form of medicine approach. Idk sad.png

Yay!!  So happy to hear. I'm just under 4 months and the increase to 75mg in the last few weeks really really made a difference.  The oil production is about 50% less, so I hope you also see that as well.  Fingers crossed the small increase helps even that much more.  I am so excited to see consistent results and hope you continue as well.  :)

Ok 6 months into Spiro and as of today my skin is clear smile.png I did get one cyst right under my lip that hurt like hell but its now gone and I don't have one single pimple! I asked my gyno to change my birth control to Ortho Tri-Cyclen and she did so I took my first pill today. I really hope the new birth control and the continued Spiro will keep me clear from here on out. I have a dermatologist appointment on Tuesday and I think he's going to up my Spiro, if he does I will ask for 75 mg and wait on going up to 100 mg. I'm really happy with my progress.

Hey there!  I upped my dose from 50 to 75mg about 2 weeks ago, still in conjunction with Yasmin. So I am at around 100mg and seeing a bit less oil.  I was SO fearful of another IB.  I did experience a small one, but honestly NOTHING like the first time at all.  It's already cleared.  I also did have a migraine followed by headache for a week and 1/2.  Drink lots of water and just be ready to deal with whatever comes.  a week or 2 of breakouts is so much easier to deal with then continual.  I wish you continued improvement!!

Hi beautiful Maria smile.png I just wanted to check in and see how you are doing. I'm about to up my dose to 100mg next week when I finally see my dermatologist after 6 months. I'm so worried about another IB I cant take anymore. The 50mg of Spiro seems to be working but not as good as I thought it should be by now. I'm still getting a cyst here and there. I've also been cutting out foods that I thought I could finally eat again like chocolate grrrr. Well, I hope the 100mg will work better for you, keep me posted on your progress smile.png

Welcome to the Spiro journey!!  Have patience and stick with it.  It does work!!

I started spiro today! She gave me a prescription for 25mg 2x/day (50mg/day) and said to start at 25mg for ~2 weeks to make sure I don't have too many side effects/to not shock my system since I seem a bit sensitive to hormone changes. And she'd bump me up to 75-100mg if there is zero change in 2 months. 


I have a mirena IUD, and now that the cramping is better and the periods are gone and I basically don't have to worry about pregnancy, I really love it. Only thing is my acne is worse on it than on the pill due to the levonorgestrel progestin in it being way androgenic, so she said the spiro+mirena combo could be a first-line and long-term option before I give up on the IUD, since it would offset the most probable cause of my acne (retin-A and antibiotics only cleared the top half of my face and it dries out my chest and back really badly). I refuse to take antibiotics long term, too. 


Fingers super crossed on this one.

Hey I am so glad to hear you are thinking of sticking it out with Spiro.  I know things have been beyond challenging for you and I hope you do see some success.  Is there anyway your dr will extend your doxy course?  When I started bcp on Spiro, I did take the packs back to back to try and keep my hormone levels regular and to try and head off any extra breakouts.  Are you on a monophasic pill, perhaps maybe you could do that as well and skip your inactive weeks.  They have done many studies that have shown it is not harmful.


I also second your choice for peels.  I've been loving them.  You may want to look into the Makeup Artists Choice Mandelic peels as they have been wonderful on my redness and even active spots.  They have an AWEsome 15% salicylic/15% Mandelic combo that worked wonders after the first time I even used it.  Dried out some spots that had been sitting around forever and really helps with the PIH.  Lactic is good for PIH but better once the active spots are minimal.  Wishing you a better week!

Week 11 (tomorrow): So, this last week or two, has been pretty good. A lot of cysts have gone down and I'm just left with a lot of redness. I'm far from cured. Still counting about 20 actives on my face alone and my shoulders are at its worst with cysts, but the really large clumps of cysts have gone down. As of right now, I told myself, if i do not get worse before my next appointment (which is in two weeks), I'm holding off on the accutane. I'm still taking the Spiro at 25mg but I think the thing that has really helped me was the doxy and the birth control pills which I started a month ago and 20 days ago for bcp. I'm really afraid of getting worse again since I'm coming to my last doxy pills and my inactive bcp both at the same time  (my luck). If I make it through this next week and I'm still under control, I decided to stick with my current treatment in addition to starting chemical peel. I've been looking up stuff online about it and think that 20% salicylic acid peels once a week would be a good place to start, since my skin should already be used to salicylic acid. I really just want to start the peels to help the redness. Im not really expecting them to do anything for the active. I'll still have bcp, spiro and doxy to help/ prevent actives. Also bought a bottle of saw palmetto. I dont really know when I should try that... thinking of starting now, since im on my inactive week of bcp. All this stuff is so confusing

Hey there!  Reading through your log and just wanted to send some positive thoughts your way.  You are nearing a month soon and that's progress.  I've been on Spiro a little over 3 months now. The first 3 months 50mg and Yasmin, now 75mg and Yasmin.  It took a few months of breakouts off and on to finally have more of a clearing phase at 3 months.  The first month was inflammed, started doxy in addition to Spiro and then it was small little comedones all on my cheeks/jawline/chin.  Slowly cleared and just have a few clogged pores on my whole face which is a slight miracle after the first month.  Exfoliation and good cleansing routine helped the process along and keep my PIH down a bit.  I did have headaches from the increase in dose, but other than that.  I don't think you will become any "worse" than you are now.  My first month was definitely a shocking one as it got way worse than I've ever broken out before.  Keep your head up and keep on your spiro journey, it does work but it takes a whole hell of a lot of patience.  Mine was tested continuously and this board and these women really helped me through it.

Yeah, the endo said 10% of women have PCOS so really do be extreme or have a mild case it still stinks for sure!!  Girl I feel you on the research part, I've been dealing with this for the past 5 months I could write a book, a blog, and launch a successful website on how to take care of acne and hormones. But I am so thankful I am armed now with all of this information and can continue to have success.  Honestly starting spiro was an easy decision, but it has been a tough journey and hoping it is so worth it.  In Nov/Dec I was getting mild but persistent breakouts, would get 1-2 spots then clear.  I decided to go on spiro to head this off, 50 mg and I had a pretty rough breakout first few weeks, inflammatory and then I went on doxy.  Then the 3-4th week it was all of these tiny comedones under my skin on my cheeks, jaw and chin, NEVER had that before in my life.  BUT my skin care routine was crazy, I was doing everything on earth to try and fix my breakouts topically before I figured out a good routine.  It was the start of winter and it was dry but oily, so as much as I want to say spiro did this, it might not have been as bad if I had my current routine down.  It's taken 3+ months to finally have some control over my skin.  It's been 2 steps forward 3 steps back but finally moving forward now.  Everyone is completely different, read the reviews.  Many women don't have an IB.  Many women do.  Some women clear quickly and some take forever.  I think if you have your diet under control, a good cleansing routine, the right dose of spiro and bc and you will be very succesful in getting control of any breakouts.  As much as this has been hell it truly is worth it to have clarity moving forward!  I have a journal on my experience:

@ Carpemomentum wow another thin cycster so to speak lol. My doctor did blood work and sonogram technically Im not a full pcos but like she said really borderline. But to be honest I consider what I have pcos Ive had every symptom accept the insulin resistance and acne. So Im sure the acne I have now will go away eventually lol. How has your experience been on spiro? Was the initial break out bad? I was considering it a while ago simply for the hirsutism which I unfortunately have been blessed with sad.png. I was hoping the right kind of bc would help regulate all of these aspects. Surely Lutera was not the one since Im sure Im sensitive to androgens as it Is and that only aggravated it. And I am going to look into ordering the Mandelic acid peel since it might be more convenient to use as opposed to stopping everything once a week with glycolic acid. But ugh I totally understand you friend its Friday night and I have been obsessively researching birth controls and hormones. I swear Ive learned so much I should have a degree by now LOL kidding of course smile.png.


@ Sandrine1985 thank you so much for the advice. I hadn't hear of pumpkin seed oil but I will definitely look into it because this oil production is causing these annoying comedones! Did it take a long time to see results?

With the mandelic acid peel, in the morning I would do my normal routine.  Wash, exfoliate, clindamycin, bp and moisutrizer and then at night the peel with just some spot treatment of bp and lotion and usually go back to my normal routine in the  am.  My skin likes the mandelic so I can be a bit tougher with it.  Actually going to try the 15% mandelic/15% salicylic tonight and excited to see if it can help clear up the final clogs left from my spironolactone/birth control breakout.  I would BE so happy if this can help these last few spots to kick the bucket.  Ugh I hate this so much.  I shouldn't be excited about doing a peel on a Friday night I should be out not worrying about my skin.  Oh well, just a few more weeks left of this and pulling to finally be clear on this combo.   I also am thin PCOS, my endo said that is really good because our fertility chances are much greater and less concern about other disease.  Either way it stinks, but spiro is great for those with PCOS. 

Carpemomentum thank you so much! I love that we can come on here and speak our minds when all of the people around us are tired of hearing it LOL! But yes Im going to try this peel out this weekend maybe. Ill stop everything including BP at least 48hrs before. Im just concerned it would might break me out a little more. Can the Mandelic one be used without stopping all topical's 48hrs prior? I am still kind of considering birth control of another kind but its so frustrating thinking I might choose something that might exacerbate the problem. II guess to describe myself Im not full blown thin pcos but when I had my blood work done I was "borderline". So all in all I would love to really just regulate my periods and body in general and hopefully not sacrifice my face or mind in the process lol. Well see what I do. Also about the diet the most Ive done is drink a little less coffee than I used to lol. Im quite thin and going on & off the pill made me loose more weight than I really wanted to. I know some people wouldn't complain about that but hey different strokes for different folks you know lol.


Hope7 also thank you very much for your input! I appreciate it a lot. Actually I made a post asking people about Fem Rebalance. It contains a lot of herbs that seem interesting. The only thing that concerns me is that at some point I took maca root and within a week I was breaking out in cystic acne. So since then Ive eliminated all supplements except for the things Ive always taken such as muilti, zinc, b12, and vitamin d.  So I don't know if I should just hold off on everything and let time do its thing or not.
