New Deep Ice Pick Scar, Suggestions For Treatment?

I'm currently on accutane and have a 1mm x 1mm hypertrophic scar right in between my eyes. It's in a terrible place, and one day when I was particlarly aggravated with it, I picked at the top a little and removed some of it. The entire hypertrophic thing became red for 1-2 days, and then a scab formed over the picked off part.


From what I've read it's that you are supposed to leave the scab on as long as possible as it is healing the area, but unfortunately I picked that off too. Two mistakes. Now there's left a pretty noticeable ice-pick above this hypertrophic thing, and the rest of this hypertrophic thing beneath it.


My dermatologist is clueless as to what to do with this, she just said she would say to inject the hypertrophic scar but is worried about creating a crater.

Any suggestions on something that is Accutane friendly to deal with this? Maybe after accutane what should I be looking at? TCA-Cross, Punch Excision? I'm primarily looking to deal with filling in the ice-pick scar. But as is my experience in the past with reading up on scars, there's no real cure, and this is more or less permanent.

Outside Temperature And Scars

Where I live, Winter is fast approaching, and today was rather cold. What I noticed when I got back inside was that my scars had improved around 80 percent (especially my rolling scars). I had that feeling of contracted skin you get when you're outside in the cold. If only there was some way to make skin always contract like that... makes me wonder if face lifts / face tightening would help aleviate acne scars. Or even if perhaps there is some matte clear kind of skin cream that bonds to your skin and to itself, so that it would hold that shape throughout the day if you apply it immediately after being outside in the cold.

Any Rolling Scar Treatments While On Accutane?

I'm currently on accutane for 4 months to finish off what's left of my acne. However, I am left with some pretty severe rolling scars on both cheeks from my many years of acne. I'll be getting subcision as soon as it's possible (6-12 months after I finish accutane), but I was wondering if there is something I can do in the mean time.

I've read that silicone sheets can improve the look of scars, is this true for rolling scars as well? Would there be any issues with constricting the air flow to my skin overnight while I wear silicone sheets?

In addition I've looked into dermaflage cover up for acne scars. It is essentially a silicone make up kit to hide imperfections. It seems like something that could work well for ice pick scars and maybe boxcar scars, however, I'm not sure if it would work well on rolling scars. There doesn't seem to be information on that. Also, the concern of clogging pores with silicone while on accutane comes up again.

So, little backstory, I've had acne from about ages 16-22 now. I've developed acne along my jawline, and I'm trying to get rid of it. I tried the acne.org regimen for one month, and it seemed to make things worse. My skin became very dry, and the BP burned my face. It also seemed to really pronounce my scars, but perhaps I was only oblivious of them before doing the regimen. Previous to that I have tried:



-I believe retin-A at some point

Currently, I'm drinking lots of water, changing my pillowcase nightly, and avoiding dairy.

My main concern is preventing future scars (as I have enough of those already).

So my questions are:

What would you recommend for the acne? I recently visited a derm and he recommended accutane.

In addition, what types of scars are these, and are they treatable?
