I agree with you. I really don't think people experience IB (hate to call it "initial breakout" since we are already broken out which is why we are on accutane) but rather a rebound from no antibiotics. I had gone off antibiotics quite a while before I started accutane so I was already breaking out when I started my course. My acne got progressively better instead of worse then better.
I'm curious, though, if there are people out there who experienced a worsening of their acne (IB?) and never took antibiotics.
I finished my course March of '07. Still cyst free. I get tiny whiteheads once in a while now and small pimples easily treated with BP. But I don't get anything embarrassing, painful or scarring. Just normal pimply pimples. About 1 every 2 or 3 weeks.
How is it going? Have you been able to keep working out? I am restarting hard workouts after being off for a year. I think I used accutane as an excuse to stop exercising hard. I still would run and walk but never really sweat, which I need to do to keep from gaining weight. You probably read that I had extreme fatigue with accutane. It really was awful. But, I am still clear and have no lingering side effects. It was like a miracle drug for me.

Hey there! I am 1 year past my last dose and still clear. I'll go on it again in a second if I even get one cyst.
The fatigue stayed for a while after I stopped but the joint pain went away. Unfortunately I did not get back into exercising so I am now flabby. Yuk. I am starting up again today, though, with a 2.5 mile jog, just to clear out the cobwebs. I need to pump up my bike tires and maybe get it turned up.
Ugh, I hate starting exercise..... But (butt) I have 2 pairs of very expensive jeans that are too tight and that is making me very motivated. That and the sun is shining here in the South. Time to tan, too.
Hi. Are you still clear? I am. It's been a year and I am thrilled. I just talked to my derm and she said if I get even one cyst I can go straight back on accutane and will probably only it once more. She's awesome and a big tane advocate. She's seen so many lives improved by it.

I read your log. I just want to let you know that accutane rocks! I am completely cleared of cystic acne after 25 years. No more pain and disfigurement. No more treatments of any kind.
I've been off of accutane since March and not one cyst.
I think accutane works on our type of acne the best. Or, it may just be the dramatic difference. Whatever the case, I see more cystic acne people on this board that have success with accutane than any other type of acne.
Good luck. Your skin will catch up to your spirit in a few short months. It's just lagging behind right now. And, even though you seem to be taking this painful and disfiguring disease in stride, you will LOVE having your pretty face back. Trust me. I hid my acne behind sarcasm and tight jeans. Well, I still love tight jeans but the sarcasm is long gone.
I now consider my skin "combination". Oily in the T zone only. I wear NO foundation at all. I still have some discoloration but it's fading. I will probably have some sort of resurfacing done in the spring, which would be a year post tane.
All side effects are gone. I still battle dry eye but my doctor does not think it's related. It just showed up worse during accutane and now I treat it with Restasis.
It really is amazing how well this stuff worked. It's a great drug and if I get even one cyst ever again, I will go back on it in a heartbeat!
