how does a sebum plug form?

i've tried dermalogicas "born to be mild" cleanser for seb.dermatits and other skin conditions and it doesnt stop the plugs...lately I've been getting these super tiny white dots that I take the end of my tweezer and stick it under the plug to almost lift it up. Sometimes I make a cut but most of the time I get the hard seed to pop up and then i can grab it. It doesnt much matter because it will come back in time in the exact same spot no matter what I put on it to prevent it. Its like a weird sort of hard keritinazation is going on. I tried more exfolaition and more moisture and it doesnt matter either. I dont get what it is. Ive been to more derms than I can count and they all look at it and say clogged pores...Clogged pores doesnt even cut it. Its not normal to go around your entire chin for hours every night sticking a tweezer in every pore and digging this shit out. Ive had this for two straight years and its getting worse.

Most people experience an initial breakout for a few weeks to a few months before they actually get better.

I... don't believe you're helping anything by pulling these things out of your face. If you provided some pictures maybe we could understand you better. There is a logical reason as to what is causing this to happen and physical extraction, given the fact that they keep returning, only seems like something that will even more aggravate your condition; after all, every time you extract something from your face you are exposing what is beneath it, which is usually just a widened pore that is prone to even more issues.

If I were you, actually, given the fact that you can pull these things out of yourself ... I would pull all of them out, exfoliate, cleanse your face by some means, and then spend a whole day of nothingness wearing some sort of mask that will allow your pores time to recover. I like to think baking soda masks help me to this end.
