I'd think you'd want to leave it on as long as possible.
My suspicion (well obviously really), if it works, it works by changing the Ph level in the pore; bacteria can't grow well in low Ph, and that's why the skin Ph is usually slightly acid: 4.5-6.5 (presumably). But washing in particular can screw that up (soap is very alkaline, that's how it kills germs).
But wacking on some vinegar and leaving it to soak in, preferably overnight should inhibit any bacterial growth in your pores. You could then wash it off in the morning.
It's interesting to me actually- I get a lot of acne around my mouth; I've noticed that it's particularly related to brushing/flossing (even sneezing

). Both saliva and toothpaste is alkaline, so I may be upsetting the balance that way. If I apply vinegar will it help? I'm certainly going to find out.
Apple vinegar might have fruit acids in it, so might work better than malt, (or not), but by all means try it, and see.
