Here is the formula you would use to figure out your dosage.

Assuming a cumulative dose of 120mg/kg (for which the majority of evidence supports as giving the best chance of remission), simply multiply your weight in kg by 120. That will give you the number of milligrams of accutane you need to take. I think most people get their accutane in 20mg tablets so if you divide that total by 20 it will tell you how many tablets you need to take over the course.

For example, if you weight 75kg, you will need to take 9000mg of accutane to reach a cumulative dose of 120mg/kg i.e. to get to this figure: 75 * 120 = 9000

Divide this by 20 (as in one tablet is 20mg) means the course will consist of 450 tablets. Lets assume you are taking 60mg a day, so three tablets. That would mean your course would be complete in 150 days which is 5 months.
Although it doesn't matter how many MG you are taking a day if you are on a lower dose you will be on the medication longer and if you are on a higher dose your course will be shorter.

My daughters ballet event last night was a total wash. Last night was supposed to be family appreciation night and they would get their awards. It also was their last practice before recitals next weekend. They were supposed to have snacks and face painting and events going on... NONE OF IT WAS THERE! So my daughter was upset because she was really looking forward to getting her face painted and all that jazz. Not to mention the teacher totally switched everything up on them. The stage they will be performing on next weekend faces the opposite way as the studio stage that they learned the dance on. So every is backwards now and the kids totally don't understand what the hell they are doing now. I am kind of pissed to be honest. 9 months they have been learning this dance and the last day of dance you switch it up on them? Plus she taught them a different ending to the dance just last night in the last 10 minutes of class. Then she gets mad when the kids didn't do it right! It was a mess. Then celebration snack she got was 1 hersheys kiss... LAME.

Thanks for posting this on the cumulative dose. I was still confused about it, too. I calculated that if I continued with 40 mg, I would be done in 5 months, but if I was bumped to 80 mg, it would be around 3 months! Dang!

That is so annoying about your daughter's recital!!!! I have a total pet peeve about people failing to communicate and then screwing up important things! Plus, if you're going to promise snacks, at least bring something delicious! What are we in, a famine? Haha

I'm sure she'll do fine!

I'm 1 month+1 week into my accutane treatment. Reading your logs and all the experiences people have been going through is a motivator for me.
I have a question, and if any gals on this thread can answer that would be great! :) Does your skin's oil production fluctuate during the month or has your skin been dry as dry since you started the treatment?
I was super oily, like OILY OILY, before starting tane. In my first week, there was minor oil. Very thin layer. After that, the oil wells in my skin dried. No oil for 3-4 weeks. Now, it's starting again. I think it could be because of my time of month? I've been taking 40mg/day since I started.

Oh that's a bit of a bummer, but maybe it's because I was oily as oily can be before starting tane.
My course is going fine so far :) My body has adjusted to it, I'm planning on continuing 40mg throughout my treatment period so no surprises happen. Current unexpected side effect is a small itchy rash on my arm, and currently a breakout on my cheeks, which I never got before tane, so even my face must not be immune to the infamous initial breakout.

I get that rash from time to time on my hands and lower arms. I just moisturize the hell out of it and it goes away. It eczema actually. I don't know why but it seems to come with taking the meds.

Hope what you're experiencing is like another initial breakout due to the increased dosage and it will go away soon.

Actually I noticed this week that my nose is a bit oily! I started on March 23rd and had been quite dry since. Pre-accutane, I had dry-combination skin and it seems to be going back to that. It's a bit strange and I don't understand why that's happening. Hopefully the medication is still working. Anyway, I'm kind of glad because it has given my red marks a chance to clear up a bit.

Betsy, this is so helpful! And pretty much exactly what my dermatologist told me last night when I basically stalked him through the phone. And that's terrible your daughter's teacher switched things up at the last minute! I teach kindergarten and one of the cardinal rules of Early Childhood (and really any) Education is that you never promise something to children unless you know for certain that you'll go through with it. And I bet the breakout is just from upping your dose to 60mg (part of the reason I'm being a wimp). Hope things get better soon :)

Ah, that's a tad bit disappointing. Good to hear it's working for you though. I had primarily oily skin that didn't break out, except for on my chest, upper arms, and back. So, the sudden shortage of oil created intensely clogged pores that made me break out on my cheeks for the first time in years. It would be cool if the oil stopped for good, and gave my skin time to purge and de-clog. Hope this is just the infamous initial breakout.

Oh you teach kindergarten! That must be so much fun. I was alittle shocked her dance teacher changed things too. They are 7-9 years old in her class. How fast can they adapt to learning something new? Plus they only got that one practice. She usually has dance Monday nights, since memorial day is Monday she doesn't have class and her recital is on sat and sun. Like wow way to give them no time! I'm kind of upset about it to be honest. They have been preparing for this dance for 9 months, I think there is plenty of time to get it down and her teacher just opted not to get things done on time. Oh well I guess there is nothing to do about it.

My skin today holy crap I have like 4 new actives today! One in the center of both my cheeks and one on each temple and the ones I got over the weekend on my forehead and jaw and still hanging tough. I'm loading on the neosporin
and hoping for the best. Kind of depressing to be dealing with the breakout again. Let's hope that this one doesn't last as long. It really sucks because all my zits from my previous breakout were all just healed and then BAM I get smacked with these guys. I have a huge red angry spot on my left cheek. I woke up with a white head on it and when I washed my face it popped open and naturally I squeezed all the gunk out of it and now it's red red red and painful. It's almost flat though so I am hoping to see improvement over night. I have a birthday party for my step sister and niece on Monday afternoon. I wanted to look somewhat decent at that considering I have to see all my dads side of the family. Also my dad told like everyone I was on this medication to clear my skin ( thanks a lot dad) but I know people will be looking and checking out the situation and judging... It's going to suck to have a bunch of angry looking ones on my face. So I'm crossing my fingers and hoping for the best.

Tip, don't mention clearing up on these logs. Bad bad luck and I break out everytime I post a ray of hope lol. Kaydon and me both seem to have bad luck with this..ugh oh well clear skin better come soon. It has to right? Has anyone seen a log or heard of someone not clearing on this? I know people might not stay clear but is it possible for this medicine not to work at all?

Aw Betsy :( Sorry to hear you're going through yet another breakout. We must have bad luck! I am waiting to wake up and see new actives chillin' on my face like "hey, sup?" Hahaha. Luckily I haven't broken out in the past few days, but I know that's likely to change! Especially after starting 80mg this Saturday, I'm pretty paranoid of having a breakout from that. We need to see some light at the end of this pimply tunnel!

I've been wondering the same thing though. Has anyone had their cumulative dosage and not replied? That's a scary thought.

Aw that makes me happy that you haven't broken out in a few days! Gives your face some time to calm down and heal up! I am pretty sure the increase in the dose is why I broke out again. I didn't think 20 mg would make much of a difference. I was wrong I guess. I hope that you respond well to the 80 and no new breakout comes along with it! I'm paranoid as ever too. With good reason though, we haven't had the easiest time so far. Very emotional at least on my part to have all this popping up and I am so far into my course! I'm just sick of this and want to move on with my life, not have acne be part of me anymore. It's depressing :(

You start your 80 on Saturday? How exciting, I am hoping to get bumped up to 80 if I'm not seeing results by the start of my 4 th month. I don't want to be on this stuff for much longer than 6 months if I don't have to. I came across a log today that the person found out there were going to be on it for 10 months and have been on 40 the whole time because thier acne isn't responding yet and they are done with month 5 if I remember correctly. That is such a long time. But I guess if I need to be on it longer than I will. No point in starting a course and not finishing it with the results you want.

My skin, well I woke up with a huge white head on my forehead begging to be popped. So I gave it what it wanted lol. Also I woke up with a nasty looking zit on my upper lip area. I just want to wake up without something new on my face for once. Last week I had two days without something new popping up. Then my dose was increased to 60 MG and BAM Im breaking out again. Hopefully this passes soon. I had no idea all this was under my skin waiting to be purged out. Nasty thing this acne is isn't it?

Awwww! I remember those years and that age. My daughter is almost 20 and my son is almost 15 - it flies by!

I am so sorry you are still breaking out at this stage of the game. My brother was on this years ago and he warned me that he did not clear until the very last month and hes had awesome skin ever since- he's in his 40s. So don't give up hope! My skin is doing okay. I am still loving the dry, not too dry just dry enough. It's been about a week since I got a new one but the two cysts on my jaw do not seem to want to budge at all. The craters are healing better thanks to the neosporin. I do have two smaller bumps on my cheek that were cysts before I started that have gone down but not all the way. They are small but I feel like they have gone down as much as they ever will and will now be there permanently - ugh! I go to the derm today and am scared to death to up my dose but I don't want to be on one that's too low. I trust her so I'll just have to take the leap if she thinks I should. I'll let you know!

Well stick with what you are comfortable with. Maybe go up to 20MG and see how you react to it. You can always lower your dose back down. Your dermatologist should be happy to write you a prescription for a lower dose if you are having problems with it. When I moved up to 60MG I broke out all over again and I have been really tired. Like 8 hours of sleep isn't enough. Yesterday I laid down on the floor to flatten out my back ( it was sore go figure) and I ended up taking a nap right then and there. Hour long nap from 4 to 5. I went to bed at 10 or 11 and woke up like are you kidding me, its time to wake up! I just can't get enough sleep. Plus its been raining and storming here so that keeps me in the house and lazy feeling which can't help the situation at all. I still want to get bummed up to 80 MG at some point if my dermatologist lets me so I guess the side effects at 60 don't bother me too much. Your dermatologist went to school for 9 years for this so I would trust her to do the right thing for you! Just tell her you concerns and feelings and she will do what is best I am sure! Good luck, Tiger!!! Go with your gut. follow your heart. It could be that a dosage increase might not matter much since you said some people clear at the end of their treatment anyways. Just a thought if you're worried about side effects. I feel you about the stubborn cysts! Grrr! I know it's too soon for me to expect them to go down, but I just thought shrinkage would be faster.

Betsy: Wow, I can totally relate! I kept thinking it was just school or life stress, but I really think it's my body adjusting to the medicine. I feel like I'm HUNGOVER when I wake up in the morning. I want to fall asleep in the car, too. Only you guys could understand how that feels. It's really weird.

I know Gina, I haven't felt like I've woken up with much energy at all since I have been on this stuff! It kind of does feel like a hangover doesn't it. Hopefully this feeling passes soon. When I was on 40 Mg I was tired in the beginning and it passed so hopefully it will pass on the 60. Some people report feeling this way the entire course, so it wouldn't shock me if it stuck around. Just hope for the best and rest when we can.
