I wanted to respond to your post because I have a very similar situation. I'm about to start accutane for the first time in a few days, but my history is pretty much the same. I have a very mild case of P.C.O.S. and was put on birth control at a very young age. It cleared my skin (which was acne prone when I went on it but not anything outrageous for a 13 year old) for years. I finally went off of it at 18 because I thought my body could use the break and my face EXPLODED. Since then I've tried every antibiotic, topical and over the counter medication out there aside from accutane.
Here is the one helpful bit of advice I've learned from that whole experience: Because our acne is hormonal, the problem is internal. I believe you actually wrote that in your initial post. That said, all of those other products (save birth control, if your find the right one, and idk about you but for me the 'right one' is always changing because so are my hormones), actually make my skin WAY worse. Right now I don't use a single acne product on my face. I wash with a mild cleanser, moisturize with jojoba oil dropped into trader joe's facial moisturizer (I highly recommend this product to ANYONE with dry, sensitive skin), and at night I remove make up with trader joe's tea tree oil cleansing pads before washing just to make sure all of my make up comes off.
Right now I have between 2 and 3 active, small cysts on my face at a time and they tend to shrink rather quickly. If i scrub, use harsh products like BP or ANYTHING like that, my face gets worse right away. I'm going on accutane because it's still very annoying that no matter what I do I still have breakouts at 22 years old, but I've discovered that in the meantime, to keep it at bay, I have to stay far, far away from the harsh acne products used to fight teenage acne or clogged pores, etc. My skin is too sensitive and it doesn't need any of that because the problem isn't external.
I definitely don't blame you for trying accutane again, I just thought I might be able to help you with a way to calm your skin down and hopefully minimize the problem in the meantime. It definitely helps me when I'm the only one who knows that I have an acne problem rather than having it written all over a bumpy, irritated face. With my process now, I can use concealer and look like a normal person who has the occasional break out.

I'm also considering going back on accutane. I also got acne back after stopping accutane. A friend of mine who is a dermatologist suggested spironolactone for me for hormonal acne. It's like every time I get that time of the month I break out and sometimes it's hard to get rid of the scarring like u were talking about. I'm also thinking of what treatment to do for my scarring so I made an appointment with my derm for next week.
