So I'm finally started on 40mg yesterday (up from 20mg for the last 2 weeks). I'm taking two 20mg pills a day (not sure if you can get a single 40mg pill?).

I took both pills at the same time and an hour later I felt a combination of very heavy and light headedness and felt pretty fatigued for the rest of the day.

Should I split the pill between day and night or should I just stick it out and take both at once and wait for my body to adjust to the increased dosage?

Even a single 20mg a day made me feel tired so I wouldn't want to feel it twice a day.The reason I prefer to take both at once is to just get it out of the way, but I'm not sure if this is the best thing to do. My derm didn't really give any advice on the matter.

What's everyone else's routine with multiple pills?

I'm on 80mg/day - 2 40mg pills a day. At first I split them up, and took one at breakfast and one at dinner. The first week I had horrible headaches but I think my body was just adjusting because they went away shortly after that.

After about 2 weeks, I started taking both pills at dinner time because it was much easier for me to remember. They also make me very tired so I've found taking them at night makes that side effect more manageable.

Hmmm, maybe I should start taking them at night then? I usually take them around 2pm so hopefully taking them around 8 hours later than normal doesn't disrupt the cycle.

Im not sure, That is a good question and one I will need to know when I up my dosage in a couple of weeks. I might call the derm and ask. Will let you know if I can get an answer this close to xmas!

I have heard they can do a 10mg tablet, so people who cant tolerate two 20mg for example can do a 20mg and a 10mg... maybe an option??

My dermatologist said that I should take them 12 hours apart as the side effects are greater when taking them both at once. The stuff stays in your system anywhere from 8-20 hours per the manufacturers website. I think you will probably be fine in a week or so if you take them separately. If it's still bothering you after the first month, then I might rethink it.

I don't think it will make much of a difference in regards to amount of side effects experienced according to when you take them. Accutane is supposed to still be in your system over a month after you finish anyway - it's why you don't get oily again immediately.

I always took it at night - just because that was my biggest meal, easier to remember etc. Plus, if it makes you sleepy, well my friend, all the better right? :)
Your body might be adjusting still, I know when I first upped my dose, I felt like my body was being fried slightly, in the sense everything was super dry feeling, I could feel the hot air rushing through my nose, and my whole body had this 'ache' feel, but you should try both out and see what works for you if you're still not sure, we're all different after all.
