I'm surprised that I can't find any other posts about sodium hydroxide. Especially since it is in Acne.org's benzoyl peroxide. I personally use BP on a regular basis, but with sodium hydroxide in the product, I'm afraid to use it.
This is mainly because sodium hydroxide (NaOH) is a caustic ingredient. Even if diluted, I feel like it can damage or dry skin out excessively. In addition, even though the product is neutralized, just the thought that hydrochloric acid is one of the main products used to create sodium hydroxide is kind of scary.
I'm not trying to cause fear, and have limited knowledge in chemistry, but I wanted to see if anybody else has any input on the subject.

It's mainly used as a pH buffer in skincare products. It is used in such a very small amount it is not irritating to the skin in most cases.
Keep in mind that you can find something scary on the internet for just about every single ingredient out there. Dan takes great care in making his products safe and effective.

the BP (a pro-oxidant) itself is worse for your skin than the small amount of that ingredient
