Well I've been doing these vitalize peels I've had two so far and the lady doing them said she'll do extractions the next time. My questions is what exactly are they? Do they hurt?

Extractions are just the removal of the material that is clogging your pores. The esthetician will either use an extraction tool or some use just their fingers wrapped in tissue. If done correctly, they should not hurt. I get them done with my facials once a month, and they really help my skin a lot.

Extraction is basically squeezing out the blackheads. She may or may not use an instrument to extract them. Yes, they do hurt. In some of my sessions, I was literally crying and had to take breaks.
Make sure the lady is very good at it. Some estheticians are not good at squeezing out the entire blackheads, which makes it worse. The key is to only extract the ones that are ready to be squeezed out and leave the other types alone that are not ready to be squeezed out in its entirety.
I once had a good esthetician squeeze the hell out of all of my blackheads on my forehead and that cleared them up for good. I dunno what she did, but she gave me a head and neck massage w/ aromatherapy to relax me and to increase blood flow to make the extractions easier. It's been about 3 years and I don't get zits on my forehead ever. No scars either. When I went back, she had since quit and the other lady was horrible.

I asked what she did to get them out and she showed me some little tool with like circlish things on the ends and she pushs down, but ya I think it's gonna hurt.

I think it partially depends on the skill of the person doing the extractions (some are gentler than others) and also your reaction to topical pain - some people are way more sensitive than others. Extractions don't bother me much at all, but I have a friend who goes to the same clinic I do and she says they hurt sometimes. But, she also says she is very sensitive to topical pain.
