Just a quickie but something i've wondered for ages, does anyone know exactly why you breakout initially when on accutane?

The Accutane is trying to dry your skin out so your body initially tries to fight against it and produces more oil!

Exfoliation, rapid exfoliation combined with a slightly changed skin structure (thinned) and decreased sebum output secondary to smaller sebaceous glands.

The exfoliation combined with the thinner skin is the biggest reason for that breakout. The same thing happens with topical retinoids...it's as if the impurities are pushed to the surface and in a sort of way, they are. It's more like they are revealed and the pores can't block up again as the skin is shedding so fast and the pore can't remain blocked and there's not enough oil to clog up the pore anymore with that rapidly shedding skin. smile.gif Or something like that.

I've been wondering the same thing - only what are the chances of an IB if you've already gone through the IB with a topical like Differin. I've been using Differin (plus BP, plus antibiotic) for 8 months now, but I'm still breaking out. I'm starting Accutane the beginning of September and wonder if the use of my current regime will prevent a bad IB or make it worse because of withdrawal. Any info. on that?

Yours might not be so bad as much of the underground infection may have already surfaced.

When I started Accutane I did not have an initial breakout at all, but then again my skin could not have gotten worse than it was at that time. biggrin.gif

Chances that you may not get the breakout is more then those who didn't use topical retinoids. I have been using Retin-A for quite some time before I started accutane and I didn't get any breakout at all. Maybe its because my dose is low but I think using retinoids before play a big part as well.

If you are still breaking out on differin, it most probably won't get worse with the use of accutane. Possibly, with accutane, your breakouts may last shorter due to the increase shedding that is more then the topical.

Well I didnt have any IB in the areas where the the Differin had cleared me. Things have been not very good everywhere else though.

I hope it gets better soon! That IB must suck. I did not experience it, but I had rather hideous cystic acne all over my entire face and I don't think it could have gotten worse. I owe Accutane a huge debt as I've never ever had cysts like that ever since.
